The autoimmunologist: geoepidemiology, a new center of gravity, and prime time for autoimmunity

There are currently over 100 human diseases that are considered to be autoimmune or chronic inflammatory affecting 5-10% of the world population and spanning through all medical specialties. As a result, health care costs are enormous and the clinical management is often challenging particularly, considering the comorbidity rates and the multi-organ involvement of each condition. We herein propose the creation of a new specialist, coined the autoimmunologist, to overcome the current limitations in the diagnostic process and clinical follow-up of patients with autoimmune diseases. More importantly, we also propose the creation of regional centers of excellence in autoimmunity where clinical research and management, as well as basic research may be united and interact in ideal synergy to ultimately create real translational research and provide better health care. Summary: The immune system plays a crucial role in maintaining tissue homeostasis and defending against pathogens and cancer cells. However, the aging process leads to a decline in immune function, resulting in compromised immune responses and unresolved inflammation. This contributes to a subclinical syndrome of homeostatic frailty in the elderly, making them more susceptible to immune debilitation and chronic low-grade inflammation, which in turn increase the risk of chronic and degenerative diseases. This review focuses on the age-dependent changes in myeloid populations and their impact on frailty and diseases in the elderly.

Antinuclear antibodies positivity in women in reproductive age: From infertility to adverse obstetrical outcomes-A meta-analysis

Summary: This systematic review and meta-analysis aimed to investigate the correlation between isolated serum antinuclear antibody (ANA) and infertility in IVF, idiopathic recurrent pregnancy losses (RPL), and pregnancy complications in the second/third trimester. The study found that ANA-positive women undergoing IVF had a lower pregnancy rate and a higher miscarriage rate compared to ANA-negative women. In terms of RPL, there was a higher prevalence of ANA-positivity. However, the effect of serum ANA on late pregnancy complications was inconclusive.

Summary: The SAVE-MORE trial found that anakinra treatment in COVID-19 pneumonia patients with suPAR levels of 6 ng/mL or more was associated with worse outcomes compared to placebo at day 28. Subgroup analyses showed that anakinra treatment had lower odds of worse outcomes in all studied subgroups. Long-term outcomes at days 60 and 90 also favored anakinra treatment.

Adverse Events to Comirnaty Vaccine Are Linked to Sex, Age and BMI: Should We Consider Reducing the Dose for Females?

Summary: An important issue often ignored is the discrepancy in medical treatment response between male and female genders. COVID-19 vaccine administration has revealed that females tend to experience more adverse events compared to males despite following identical protocols. Through logistic regression analysis of adverse events in 2385 healthcare workers receiving the Comirnaty vaccine, we found that age, sex, COVID-19 history, and BMI may contribute to the development of adverse events, particularly in young subjects, females, and individuals with a BMI below 25 kg/m(2). Furthermore, partial dependence plots indicate a 50% probability of mild adverse events lasting for a long period (>= 7 days) or any duration of severe adverse events in women below 40 years old with a BMI < 20 kg/m(2). Given that this effect is more prominent after the second vaccine dose, we propose adjusting the vaccine dosage for additional booster doses based on age, sex, and BMI to reduce adverse events without compromising vaccine efficacy.

The mechanisms and predictors of RA-ILD are limited, and early identification is crucial for timely treatment. New biomarkers, imaging techniques, and diagnostic pathways are needed for better management of RA-ILD.

Interstitial lung disease associated with inflammatory myositis: Autoantibodies, clinical phenotypes, and progressive fibrosis

Summary: Progressive pulmonary fibrosis is diagnosed when interstitial lung disease progresses without any other causes, and some myositis patients with associated interstitial lung disease may develop this condition. Autoantibodies increase the risk of pulmonary fibrosis in myositis, and serum biomarkers can predict pulmonary involvement and the early identification of progressive pulmonary fibrosis. The association between anti-Ro52 and pulmonary fibrosis in patients with inflammatory myositis has been supported by previous evidence.

Summary: This study assessed the efficacy of belimumab in treating joint and skin manifestations in a nationwide cohort of SLE patients. The results showed that belimumab significantly improved joint and skin symptoms and reduced the use of glucocorticoids. A significant proportion of patients who did not achieve complete remission initially achieved remission during follow-up.

The clinical presentation involves embolic events, with fewer valvular complications compared to infective forms of endocarditis. The diagnosis is primarily based on echocardiography, with comprehensive microbiological and serological tests to exclude infective endocarditis.

Hepatic and pulmonary involvement in a patient with PR3-ANCA vasculitis following SARS-CoV-2 vaccination: A case report

Summary: We report the first case of ANCA-associated vasculitis following Pfizer-BioNTech vaccine, with prominent liver involvement and alveolar haemorrhage. The patient developed inflammatory arthralgias and hypertransaminasaemia two weeks after vaccination. Fever and haemoptysis appeared two months later, and the patient tested positive for anti-proteinase 3 autoantibodies.
