Summary: The meta-analysis aims to evaluate and compare the impact of tobacco usage on surgical site wound problems (SSWPs) after primary total hip and total knee arthroplasty (PTH & TKA). A total of 15 studies from 2001 to 2023 were analyzed, including 560,819 individuals with PTH & TKA. The results showed that smokers had significantly higher SSWPs compared with non-smokers in this population.

Summary: This study found an increased risk of deep surgical site infections (SSIs) in patients with psoriasis after total knee arthroplasty (TKA). Therefore, increased pre-operative counseling, shared decision-making, and infectious precautions are necessary in the TKA population with psoriasis, and attention should be paid to the comorbidities of psoriasis patients.

Summary: This study aimed to evaluate the reduction in complications after total hip arthroplasty (THA) for morbidly obese and non-morbidly obese patients. The results showed that morbidly obese patients had a higher risk of readmission, reoperation, and infectious complications. Although morbidly obese patients still had a higher risk of complications, there does not appear to be a significant increase in outcome disparity between morbidly obese and non-morbidly obese patients.

Summary: This case report describes a complication of extensive skin necrosis after total hip arthroplasty, but without joint infection. The patient underwent debridement and tissue reconstruction, and achieved satisfactory outcomes with pain-free hip and restored motion after 1 year.

Summary: This study evaluates the peri-operative outcomes of total hip arthroplasty (THA) with patient-specific instrumentation (PSI) graft in patients with developmental dysplasia of the hip (DDH). The study shows that THA with PSI graft provides a safe procedure without significant complications, potentially increasing acetabular coverage rates and graft union rates. Further studies are required to confirm these findings.

Summary: The study found that younger surgeons (<45 years) had the highest rate of surgical complications following primary total hip arthroplasty in Ontario, while surgeons aged 45-55 had the lowest rate. Older surgeons also had higher complication rates, but when excluding low-volume surgeons, older high-volume surgeons had similar complication rates to middle-aged surgeons.

Summary: This study aimed to identify mechanical failures in total hip arthroplasties performed at a major academic center and their associated risk factors. The results showed that there were different types of mechanical failures in 13 patients, and they were not related to participation in sports or BMI. Therefore, it is important to assess and screen patients for risk factors before surgery.

Summary: This study investigated the relationship between upper-thigh intermuscular fat, subcutaneous fat measured by CT, and major complications after primary THA. Results showed that increased upper-thigh intermuscular fat was associated with a higher risk of major complications. The multivariable Cox regression analysis revealed that each standard deviation increase in intermuscular fat area was associated with an increased risk of total major complications.

Summary: This study describes a simplified technique for abductor repair using the transfer of gluteus maximus (Gmax) and tensor fascia lata (TFL) in revision total hip arthroplasty (THA). Although patients reported improved function after the reconstruction, there was still mild to moderate limping. Further research is needed to determine the efficacy of this technique and patient reported outcomes.
