Magnetic Resonance Imaging Findings in Painful Metal-On-Metal Hips A Prospective Study

Metal artifact reduction sequence magnetic resonance imaging findings are reported in a prospective series of 31 patients with unexplained painful metal-on-metal (MOM) hips. The abnormalities identified were fluid collection (20 patients), solid mass (2 patients), moderate to severe muscle atrophy (23 patients), and muscle edema (8 patients). In conclusion, soft tissue lesions and muscle atrophy appear to be prevalent in unexplained painful MOM hips. Metal artifact reduction sequence magnetic resonance imaging may be useful to diagnose and monitor at-risk hips but requires validation in well-functioning MOM hips before it can guide clinical decision making.

Self-Reported Neurotoxic Symptoms in Hip Arthroplasty Patients With Highly Elevated Blood Cobalt: A Case-Control Study

Summary: This study investigated the prevalence of self-reported neurotoxicity and cognitive defects in hip replacement patients with markedly raised blood cobalt. Metal-on-metal total hip arthroplasty: does increasing modularity affect clinical outcome?

Rod Fracture in Magnetically Controlled Growing Spine Rods

Summary: Retrospective analysis of explanted magnetically controlled growing rods (MCGRs) aimed to understand their fracture mechanisms and related risk factors. Findings showed that all fractured rods were single-rod constructs and greater rod contouring angles and distances between anchoring points were significant factors contributing to the fractures. Therefore, implanting double rod constructs in patients with mature spines is preferable to avoid this complication.

Statistical Shape Modelling the In Vivo Location of Acetabular Wear in Retrieved Hip Implants

Summary: This study aimed to identify the most common locations of wear in vivo by using CT imaging, retrieval analysis, and statistical shape modelling (SSM). The results showed that wear volume, positioning, time, gender, and size had an impact on the location of wear. Understanding the function of hip implants can inform future design and improve the safe zone for implant positioning.

Summary: Physical activity and a healthy lifestyle are important for delaying sarcopenia. This study aimed to quantify the benefits of cycling on muscle health in middle-aged men. Statistical shape modeling of the large acetabular defect in hip revision surgery

Summary: The assessment of three-dimensional bony defects is important for surgical planning of hip reconstruction. Previous methods using mirroring of the contralateral side may not be useful when diseased or replaced. Statistical Shape Models (SSMs) can aid reconstruction by deriving landmarks for patients with large acetabular defects. Our study found that SSMs can effectively reconstruct bony landmarks in patients with significant lysis, regardless of defect severity. This can assist in surgical planning and implant design for hip reconstruction.

Camouflage uncovered: what should happen next?

Summary: Recent publications have highlighted the possibility of significant performance variations among different brands of joint replacement. The National Joint Registry has conducted a detailed analysis on larger families of knee arthroplasties, with the aim of identifying any hidden differences. Analysis of the Nexgen knee arthroplasty brand revealed particularly high revision rates for aseptic loosening of the tibia in certain posterior-stabilized combinations, leading to a recall of the Nexgen 'option' tibial component. More sophisticated signal detection methods are needed to identify such variations in outcomes between closely related implant types in order to protect patients.

Periprosthetic acetabular fractures as a complication of total hip arthroplasty

Summary: Periprosthetic acetabular fractures are rare but potentially serious complications of total hip arthroplasty. There is a lack of literature on these fractures compared to periprosthetic femoral fractures after total hip arthroplasty. This article provides an up-to-date review of the epidemiology, risk factors, diagnostic challenges, classifications, and management strategies for periprosthetic acetabular fractures after total hip arthroplasty.

Overcoming floor and ceiling effects in knee arthroplasty outcome measurement

The variability of CT scan protocols for total hip arthroplasty: a call for harmonisation

Summary: CT is commonly used for pre-operative planning and assessment of THA, but there is a need for a harmonised low-dose protocol due to the variability in existing protocols. Most protocols are incomplete and only a small percentage are optimised for both field of view and image acquisition parameters. The variability emphasizes the need for a more consistent and optimal protocol for THA planning.
