Population pharmacokinetics of daptomycin in critically ill patients with various degrees of renal impairment

Objectives: The objective of this study was to characterize the pharmacokinetics of unbound and total concentrations of daptomycin in infected ICU patients with various degrees of renal impairment. From these results, the probability of attaining antimicrobial efficacy and the risks of toxicity were assessed. Methods: Twenty-four ICU patients with various renal functions and requiring treatment of complicated skin and soft-tissue infections, bacteraemia, or endocarditis with daptomycin were recruited. Daptomycin (Cubicin (R)) at 10 mg/kg was administered every 24 h for patients with creatinine clearance (CLCR) >= 30 mL/min and every 48 h for patients with CLCR <30 mL/min. Total and unbound plasma concentrations and urine concentrations of daptomycin were analysed simultaneously following a population pharmacokinetic approach. Simulations were conducted to estimate the probability of attaining efficacy (unbound AUC(u)/MIC >40 or >80) or toxicity (C-min >24.3 mg/L) targets. Results: Exposure to unbound daptomycin increased when the renal function decreased, thus increasing the probability of reaching the efficacy targets, but also the risk of toxicity. Modifications of the unbound fraction (f(u)) of daptomycin did not affect the pharmacokinetics of unbound daptomycin, but did affect the pharmacokinetics of total daptomycin. Conclusions: Daptomycin at 10 mg/kg q24h allowed efficacy pharmacokinetic/pharmacodynamic targets for ICU patients with CLCR >= 30 mL/min to be reached. For patients with CLCR <30 mL/min, halving the rate of drug administration, i.e. 10 mg/kg q48h, was sufficient to reach these targets. No adverse events were observed, but the toxicity of the 10 mg/kg q24h dosing regimen should be further assessed, particularly for patients with altered renal function.

Summary: In French ICUs, midazolam and sufentanil are often used for sedation/analgesia in patients with brain injury, instead of alternative drugs. The use of cerebral monitoring devices is insufficient, although efforts should be made to emphasize the use of multiple monitoring modes and adherence to indications for sedation to improve care for these patients. Nursing use of sedation and pain scales could help reduce the dosages of midazolam and sufentanil administered in management of patients with brain injury.

Modelled Target Attainment after Temocillin Treatment in Severe Pneumonia: Systemic and Epithelial Lining Fluid Pharmacokinetics of Continuous versus Intermittent Infusions

Summary: This article describes the population pharmacokinetics of temocillin administered via continuous infusion versus intermittent infusion in critically ill patients with pneumonia. The study also examines the penetration ratios of the drug in the epithelial lining fluid/plasma and determines the probability of achieving target concentrations for a range of MICs. The results show that continuous infusion provides better PK/PD indexes but still falls below recommended doses for systemic infections, unless the patient has moderate renal impairment.

The Role of Catecholamines in Pathophysiological Liver Processes

Summary: In recent years, there has been an increasing number of research publications on the role of catecholamines in liver diseases. However, the mechanisms behind these effects are not well understood. This review focuses on the interaction between the liver and catecholamines, the expression of adrenergic and dopaminergic receptors in liver cells, and the involvement of catecholamines in physiological and pathophysiological processes. The importance of understanding these mechanisms for potential therapeutic use of adrenergic and dopaminergic antagonists is highlighted.

Pharmacokinetic and pharmacodynamic properties of polymyxin B in Escherichia coli and Klebsiella pneumoniae murine infection models

Summary: In this study, the pharmacokinetics and pharmacodynamics of polymyxin B were determined in neutropenic infected mice. The fAUC/MIC index was found to best describe the drug’s effectiveness. However, the standard dosing regimen did not achieve the PK/PD targets for the majority of clinical isolates, suggesting that polymyxin B may be ineffective as monotherapy for serious infections.

These recommendations aim to optimize the anaesthetic management for thrombectomy in patients suffering from stroke and were agreed upon by experts.

Influence of User Posture and Virtual Exercise on Impression of Locomotion During VR Observation

Summary: This paper shows that the impression of walking in Virtual Reality (VR) can be extended to other postures and locomotion exercises. Two user studies were conducted with participants wearing a VR headset and observed a first-person avatar performing virtual exercises. The results indicate that participants experienced the impression of locomotion regardless of their posture, but a specific posture significantly affected their embodiment and perception of locomotion.

Effect of Vibrations on Impression of Walking and Embodiment With First- and Third-Person Avatar

Summary: This study investigates how underfoot vibrotactile feedback can enhance the impression of walking and embodiment in static users. The results show that both constant and phase-based vibrotactile rendering significantly improve the walking impression in both first and third-person perspectives. Interestingly, phase-based rendering has a greater impact on the walking impression in the third-person perspective. Furthermore, receiving vibrotactile feedback improves embodiment in both perspectives of the avatar.
