KPC-2-Producing Carbapenem-Resistant Klebsiella pneumoniae of the Uncommon ST29 Type Carrying OXA-926, a Novel Narrow-Spectrum OXA beta-Lactamase

Summary: A carbapenem-resistant Klebsiella pneumoniae (CRKP) clinical strain carrying a novel bla(OXA-926) gene and belonging to the rare ST29 type was isolated from patient blood. The strain harbored a 94.9-kb self-transmissible IncFII plasmid with bla(KPC-2). Phylogenomic analysis of the ST29 complex identified 157 genomes. This complex is commonly linked to extended-spectrum beta-lactamase-encoding genes, particularly bla(CTX-M-15), but rarely carries carbapenemase genes. The novel plasmid-borne beta-lactamase bla(OXA-926) confers reduced susceptibility to specific beta-lactams and does not fully respond to avibactam, suggesting its potential origin from a species related to Variovorax guangxiensis >10 years ago.

Conjugation of a Hybrid Plasmid Encoding Hypervirulence and Carbapenem Resistance in Klebsiella pneumoniae of Sequence Type 592

Summary: Klebsiella pneumoniae strains carrying both carbapenem resistance and hypervirulence genes pose a severe threat to human health. This study reports a novel type of hvCRKP strain, which emerged from the carbapenem-susceptible hypervirulent K. pneumoniae lineage and exhibits unique characteristics compared to other strains. The findings reveal a new mechanism of genetic transfer and emphasize the need for rigorous monitoring and novel countermeasures to curb plasmid-mediated transmission of hvCRKP.

Antimicrobial Resistance Conferred by OXA-48 β-Lactamases: Towards a Detailed Mechanistic Understanding

Summary: OXA-48-type beta-lactamases are commonly encountered in bacterial infections caused by carbapenem-resistant Enterobacterales. The enzymes have a growing array of variants with varying activity spectra, and research is beginning to uncover the underlying basis for these differences and the resulting resistance phenotypes. Future studies will focus on understanding the activity spectra of these variants and their interactions with antibiotics and inhibitors.

Specific Amino Acid Substitutions in OXA-51-Type β-Lactamase Enhance Catalytic Activity to a Level Comparable to Carbapenemase OXA-23 and OXA-24/40

Summary: This study aimed to investigate the effect of key amino acid substitutions in OXA-51 variants of carbapenem-hydrolyzing class D beta-lactamases (CHDLs) on substrate catalysis. Mutational and structural analyses revealed that specific amino acid substitutions can increase catalytic activity, with the I129L mutation having the most significant effect. Additionally, the combination of W222G and I129L substitutions in OXA-66 showed a strong catalytic enhancement effect, surpassing other variants in activity against carbapenems. The blaO(XA-51) gene in Acinetobacter baumannii is likely to continue evolving, generating mutant genes that encode carbapenemase with extremely strong catalytic activity.

Summary: This study investigates the association between CRISPR/Cas systems and antibiotic resistance in Klebsiella pneumoniae isolates from Iraqi patients. The results show an inverse correlation between the absence of CRISPR/Cas systems and antibiotic resistance. Bacteria are more likely to develop multidrug resistance in the absence of CRISPR/Cas systems.

This study paves the way for new approaches to treating resistant A. baumannii strains.

First Report of blaNDM-1 Bearing IncX3 Plasmid in Clinically Isolated ST11 Klebsiella pneumoniae from Pakistan

Summary: The study revealed the presence of various carbapenemases-producing K. pneumoniae strains in clinical samples from Pakistan, with a high prevalence of those carrying the bla(NDM-1) gene. These strains with different sequence types also showed some variation in cluster analysis. This study also identified IncX3 plasmids carrying the bla(NDM-1) gene for the first time in Pakistan, suggesting a potential new vehicle for disseminating bla(NDM-1) among Enterobacteriaceae due to its high transferability rate.

Whole-Genome Sequencing (WGS) of Carbapenem-Resistant K. pneumoniae Isolated in Long-Term Care Facilities in the Northern Italian Region

Summary: This study identified thirteen K. pneumoniae lineages spreading among long-term care facilities in Northern Italy, with the prevalence of ST307, ST512, and ST37 as major lineages. The most widespread resistance genes in these facilities were bla(KPC-2), bla(KPC-3), bla(KPC-9), bla(SHV-11), bla(SHV-28), bla(CTX-M-15), bla(OXA-1), bla(OXA-9), bla(OXA-23), qnrS1, qnrB19, qnrB66, aac(6 ')-Ib-cr, and fosA. Additionally, KPC carbapenemases were found to be the most common beta-lactamases.

Detection of carbapenemase-producing, hypervirulent Klebsiella spp. in wastewater and their potential transmission to river water and WWTP employees

Summary: This study aimed to analyze the outer membrane porin-encoding genes in OXA-48 producing Klebsiella pneumoniae and assess their impact on ceftazidime/avibactam susceptibility, as well as to describe a screening methodology. Various OmpK36 alterations were detected in OXA-48-producing K. pneumoniae isolates, with high-level meropenem resistance. All isolates were susceptible to ceftazidime/avibactam, suggesting its potential as a choice therapy for these challenging isolates.

Emergence of ST39 carbapenem-resistant Klebsiella pneumoniae producing VIM-1 and KPC-2

Mechanistic Basis of OXA-48-like β-Lactamases' Hydrolysis of Carbapenems

Summary: Carbapenem-hydrolyzing class D beta-lactamases (CHDLs) confer resistance to carbapenem antibiotics, with OXA-48 and OXA-163 being members of the OXA-48-like enzyme group. OXA-163 displays altered substrate specificity compared to OXA-48, with decreased carbapenemase activity due to a slower deacylation reaction. Structural analysis suggests that differences in active site architecture contribute to the differences in carbapenem hydrolysis between OXA-48 and OXA-163.

Innovative biocomposite development based on the incorporation ofSalvadora persicain acrylic resin for dental material

Summary: In this study, a novel biocomposite with poly(methyl methacrylate) as resin containing Salvadora persica powders was developed to enhance the bioactive performance of dental restoration materials. Material characterization showed the presence of organic and mineral chemicals compounds of S. persica in the dental material, which are responsible for biological activities and dental applications. The study also proved the absence of toxic residual monomers and demonstrated the antioxidant and antibacterial activities of the composite.

Summary: The study compared the relative value of conventional molecular methods and whole-genome sequencing in subtyping Salmonella enterica serovar Enteritidis in Tunisia from 2000 to 2015, and investigated the genetic diversity of this serotype. WGS showed excellent discriminatory power and demonstrated added value as a complementary subtyping method for discriminating outbreak from non-outbreak isolates belonging to common subtypes. Continuing the survey of Salmonella Enteritidis lineages using WGS in Tunisia is important.

Clinical optochin resistant Streptococcus pneumoniae and Streptococcus pseudopneumoniae strains in Tunisia

Summary: This study identified atypical pneumococci using multiple methods and identified several Optr S. pneumoniae strains and one Streptococcus pseudopneumoniae. The results indicate that bile OD-values can be used as a rapid method to discriminate between these strains.

Relationship Between Serotypes and Antimicrobial Nonsusceptibility of Streptococcus pneumoniae Clinical Isolates in Tunisia

Summary: In Tunisia, a study was conducted to determine the relationship between serotypes and antimicrobial nonsusceptibility of Streptococcus pneumoniae isolates recovered from clinical samples prior to the introduction of PCV10 in the national immunization program. The study found high rates of penicillin nonsusceptibility and multidrug resistance among the isolates, with associations noted between specific serotypes and these resistance patterns. The results highlight the importance of monitoring the impact of the newly introduced PCV10 on pneumococcal antimicrobial resistance in Tunisia.

Summary: In this study, the prevalence of plasmid colistin resistance in clinical bacteria in Tunisia was investigated. A total of 836 Gram-negative bacteria strains were tested, with 5.02% of the isolates showing resistance to colistin. The mcr-1 gene was detected in 0.59% of E. coli isolates causing urinary tract infections, accompanied by other resistance genes such as bla(TEM-1) and bla(CTX-M-15).
