Intracranial pressure monitoring in patients with acute brain injury in the intensive care unit (SYNAPSE-ICU) : an international, prospective observational cohort study

Chiara Robba, Francesca Graziano, Paola Rebora, Francesca Elli, Carlo Giussani, Mauro Oddo, Geert Meyfroidt, Raimund Helbok, Fabio S. Taccone, Lara Prisco, Jean-Louis Vincent, Jose Suarez, Nino Stocchetti, Giuseppe Citerio

Summary: This study investigated the practice of intracranial pressure (ICP) monitoring in acute brain injury patients and its impact on patient outcomes. The results showed significant variability in ICP monitoring use across different centers, with a potential association between ICP monitoring and lower 6-month mortality and better long-term neurological outcomes in severe cases.

Kwa, Sadia Shakoor, Ashika Singh-Moodley, Olga Perovic, Jan Jacobs, Octavie Lunguya, Raphael Simon, Alan S. Cross, Sharon M. Tennant

Summary: This study identified the common O antigen and flagellin types, as well as antibiotic resistance profiles, of 413 Pseudomonas aeruginosa isolates from patients in ten countries across four continents. Vaccines targeting the common O antigens and two flagellin antigens would be an excellent strategy to prevent invasive P. aeruginosa infections.

Allele-specific collateral and fitness effects determine the dynamics of fluoroquinolone resistance evolution

Apostolos Liakopoulos, Linda B. S. Aulin, Matteo Buffoni, Efthymian Fragkiskou, J. G. Coen van Hasselt, Daniel E. Rozen

Summary: Collateral sensitivity (CS) is a phenomenon where resistance to one antibiotic increases sensitivity to other antibiotics, providing treatment opportunities to combat antibiotic resistance. This study investigates the generality of CS effects for different resistance mutations in Streptococcus pneumoniae, a gram-positive pathogen. By measuring the collateral and fitness effects of relevant resistance alleles, the researchers identify common CS effects, but also unique effects for each allele or allelic pair. Mathematical modeling demonstrates that both collateral and fitness effects impact the population dynamics of resistance evolution. The findings highlight the importance of considering allelic identity and interactions in CS-based therapies.

Phenotypic and molecular characterization of blaCTX-M-3 and blaCTX-M-55-producing monophasic Salmonella enterica serovar Typhimurium in Greece

Efthymia Protonotariou, Georgios Meletis, Theofilos Papadopoulos, Georgia Kagkalou, Areti Tychala, Marie Anne Chattaway, Georgia Mandilara, Satheesh Nair, Lemonia Skoura

Summary: This study characterized three strains of Salmonella enterica serovar Typhimurium using whole genome sequencing and conventional methods. The strains showed multiple drug resistance, including the emergence of extended spectrum ss-lactamase (ESBL) in one strain, making treatment difficult.

First detection of mcr-1-producing Escherichia coli in Greece

Summary: This study describes the emergence of the mcr-1 colistin resistance gene in a blaCTX-M-32 extended-spectrum-beta-lactamase-producing Escherichia coli isolate recovered from a pediatric patient in Greece. This is the first detection of mcr-1 in a human specimen in Greece, and its potential spread in the country is concerning due to the existing high rates of carbapenem resistance and colistin usage as a last resort regimen.

Longitudinal Study of Dynamic Epidemiology of Extended-Spectrum Beta-Lactamase-Producing Escherichia coli in Pigs and Humans Living and/or Working on Pig Farms

Wietske Dohmen, Apostolos Liakopoulos, Marc J. M. Bonten, Dik J. Mevius, Dick J. J. Heederik

Summary: A long-term study in 39 Dutch pig farms revealed the presence of ESBL-producing Enterobacterales in both pigs and pig farmers, suggesting the possibility of genetic and horizontal transmission. These findings are crucial for developing targeted intervention strategies to reduce the risk of human exposure to ESBL-producing Enterobacterales.

Summary: The prompt detection of carbapenemases among Gram-negative bacteria is vital for infection control. This study evaluated a new immunochromatographic assay for detecting carbapenemases and found it to be highly effective and faster than traditional methods. The assay detected more carbapenemases than the standard test, especially in certain types of bacteria.
