The role of efavirenz compared with protease inhibitors in the body fat changes associated with highly active antiretroviral therapy

Highly active antiretroviral therapy plays a central role in the development of lipodystrophy syndrome, which may affect up to 50% of patients depending on the diagnostic criteria used. Most protease inhibitors and nucleoside reverse transcriptase inhibitors (NRTIs) are involved in body fat changes and associated metabolic disturbances. In contrast, non-NRTIs have not been directly related to the onset of this syndrome. One of the most widely used methods to evaluate body fat changes is dual-energy X-ray absorptiometry (DEXA), which can detect differences in the distribution of body fat in patients with and without lipodystrophy. New information from a randomized open-label clinical trial suggests that efavirenz could have greater potential for causing lipoatrophy than lopinavir+ritonavir. This paper examines the impact of efavirenz on adipose tissue and body fat composition in order to evaluate whether this drug plays a role in the development of lipodystrophy. We have focused on the evidence obtained from comparative randomized clinical trials that use an objective measurement of fat distribution, such as DEXA. We analysed available in vitro data and evidence from non-comparative clinical trials.

Summary: We assessed the prevalence of active HCV infection among people living with HIV (PLWH) in Spain in 2019 and compared the results with those of four similar studies performed during 2015-2018. The prevalence of active HCV infection in PLWH was 2.2% in 2019, which was 90% lower than in 2015.

Summary: Post-kala-azar dermal leishmaniasis caused by Leishmania infantum in an HIV-negative patient is rare, and this case highlights its association with other types of immunosuppression. In addition, this case report presents an effective therapeutic option with miltefosine, which can be very useful due to limited reported experience.

Summary: This study describes the microbiological, clinical, and epidemiological characteristics of imported rickettsioses in travellers and migrants in Spain. The results show that there were 54 cases of imported rickettsioses from 2009 to 2020, with most patients being male and a median age of 37 years. The most common symptoms were febrile syndrome and cutaneous manifestations. Most diagnoses were confirmed by serology, and only a few cases required hospitalization. This study highlights the importance of considering rickettsial diseases in travellers from high-risk areas who have engaged in risky activities.

Acute Bacterial Meningitis in Healthy Adult Patients: A Prospective Cohort Study

Summary: Human papillomavirus can lead to HSILs in the anogenital area, and the microbiome may play a role. HIV-MSM individuals have a high risk of anal cancer, but current screening methods for HSILs lack specificity. This study found that elevated levels of microbiome-encoded proteins were associated with HSILs, and measuring these biomarkers improved the current strategy of anal cancer screening.

Summary: We investigated the clinical outcomes of measles in people with and without HIV in Catalonia, Spain, and found that vaccination had a significant impact. People with HIV, unless immunosuppressed, were not at increased risk of severe disease or hospitalization. The dose-sparing smallpox vaccination effectively reduced measles incidence in both populations.

Is It Time for Treatment as Prevention of Chronic Hepatitis B?

Summary: Hepatitis B is a major global health problem, with a large number of people carrying the virus and continuing transmission. Current treatment guidelines do not include many chronically infected patients or pregnant women, making it difficult to control the spread. Expanding and facilitating treatment as a public health intervention could help achieve the United Nations' goal for 2030.

Summary: The aim of this study was to describe the mortality rate of non-AIDS-defining cancer (NADC) among people living with HIV (PLWH), compare it with the general population, and assess potential risk factors. The study found that NADC is an important cause of death among PLWH, with an increased risk associated with immunodeficiency, smoking, and coinfections. This highlights the need for early detection and treatment of cancer in this population.
