Summary: This study investigated the prevalence and factors associated with the initiation of oral anticoagulation among patients with acute ischemic stroke and concurrent atrial fibrillation in China. The results showed that less than half of the patients initiated oral anticoagulation at discharge, and only 20% of high-risk stroke patients had been using anticoagulants prior to the onset of stroke. This highlights a significant care gap in hospitalized stroke patients and emphasizes the importance of atrial fibrillation management.

Summary: The frequency of ischemic strokes (IS) in patients taking non-vitamin K antagonist oral anticoagulants (NOACs) is increasing. This study identified clinical, echocardiographic, and neuroimaging markers associated with IS in patients using NOACs for atrial fibrillation. Risk factors for developing IS among NOAC users included prior cerebrovascular events, diabetes, left ventricular hypertrophy, and increased left atrial size.

Summary: Remote ischemic conditioning (RIC) has shown variable results in the treatment of cerebrovascular and cardiovascular disease. While milestone studies suggest limited roles for RIC in cardiovascular disease patients, recent large trials found promising results in cerebrovascular disease patients. This article discusses important clinical trials of RIC in cardio-cerebrovascular disease and highlights the challenges of clinical translation. It proposes several promising research directions that should be investigated before RIC can be applied in clinical practice.
