Genetic influence on the age at onset of asthma: A twin study

Background: Although the genetics of asthma susceptibility have been frequently explored, little is known about genetic factors that influence the age at onset of asthma. Objective: To study the variation in the age at onset of asthma attributable to genetic and environmental factors. Methods: Data on the age at onset and predictors of asthma were collected in 2002 via a multidisciplinary questionnaire study of 34,782 Danish twins 20 to 71 years of age. Survival analytic methods were applied to partition variation in the age at onset of asthma into genetic and environmental components. Results: Sex, hay fever, atopic dermatitis, smoking, and exposure to passive smoking in childhood were significant risk factors, whereas BCG vaccination was protective for asthma. The risk of asthma in the co-twin of an affected twin was higher in monozygotic than in dizygotic twins (hazard ratio, 2.59; 95% CI, 1.83-3.68; P < .001). The risk of asthma in the co-twin decreased with increasing age at onset of asthma in the index twin (hazard ratio per ten years, 0.86; 95% CI, 0.76-0.98; P = .019). The effect was attenuated in dizygotic twins relative to monozygotic twins (P = .005). Genetic factors explained 34% of the variation in the age at onset of asthma, and environmental factors accounted for 66%. Conclusion: Host-related differences in genetic makeup cause different individuals to develop asthma at different ages. Better understanding of the causes for variation in the age at onset of disease may ultimately lead the way to targeted treatments. (J Allergy Clin Immunol 2010;126:626-30.)

Summary: This study aimed to investigate the co-existence of small airways disease (SAD) and upper airway symptoms in patients with chronic obstructive pulmonary disease (COPD). The results showed a high prevalence of SAD in COPD patients, but no association between upper airway symptoms and small airways disease was found.

High Use of Antidepressant Medication in Both Mild-to-Modelate and Possible Severe Asthma - A Nationwide Cohort Study

Summary: The purpose of this study was to investigate the prevalence and risk factors for antidepressant use in asthma patients compared to the background population. The results revealed a significantly higher use of antidepressant drugs in asthma patients, even after adjusting for known risk factors. Female sex, higher age, being divorced or never married, having lower education level and lower income were associated with higher odds of antidepressant use. The study suggests that living with asthma carries a psychological burden, leading to increased use of antidepressants.

Summary: Using genetic twin design, this study analyzed the genetic correlations of BMI and height in children and adolescents, finding that the genetic correlations of BMI decreased with increasing age between assessments, especially from early to middle childhood, while the genetic correlations of height remained strong across all ages.

Summary: Exercise capacity in severe asthma is associated with asthma control and BMI. In females, asthma control and BMI are significantly associated with exercise capacity. Summary: The 2023 Prohibited List permits athletes to use a standard dose of 25 mu g daily of the beta(2)-agonist vilanterol. However, there is limited data on its urine pharmacokinetics, making it challenging to distinguish therapeutic use from supratherapeutic use. A study was conducted on 25 trained individuals to examine the urine concentrations of vilanterol and its metabolites after inhalation of different doses. The findings suggest that current urine concentration thresholds are insufficient, but a higher decision limit of 6 ng center dot ml(-1) is proposed to minimize the risk of false-positive results while still detecting supratherapeutic use.

Summary: In this study, we compared the distances between visual cortical areas and overlying dura mater in female patients with migraine aura without headache and female patients with migraine aura with headache. The results showed no significant differences in distances or volumes between the two groups. Therefore, we conclude that there may not be a direct connection between migraine aura and headache.

Summary: The current framework for testing and regulating vaccines needs to be updated to consider the non-specific effects of vaccines, which can affect all-cause mortality and morbidity in ways that are not explained by the prevention of the targeted disease. Immunological studies have provided mechanisms that explain how vaccines might modulate the immune response to unrelated pathogens. Non-specific effects are often greater in female individuals. Currently, these effects are not routinely captured in clinical trials or post-licensure safety surveillance, but a new framework is proposed to address this issue.

General practitioners' assessment of interventions applied to optimize laboratory test utilization: a cross-sectional survey study

Summary: GPs in the Region of Southern Denmark were randomly assigned to interventions aimed at maximizing their use of Vitamin D tests. A survey study was conducted to examine GPs' assessment of these interventions. The results showed that guidelines were found helpful by half of the GPs and feedback reports were often read. Pop-up alerts were accepted for frequently used tests, but there were concerns about the interruptive alert appearing every time a Vitamin D test was requested.
