Summary: This study examined the effect of the second-generation antipsychotic quetiapine on ACC activity in major depressive disorder patients. The results showed that patients taking quetiapine had significantly higher activity in the ACC during reward stimulation compared to those not taking antipsychotic medication. This suggests that the antidepressant effects of quetiapine may be mediated through increased ACC activity during reward processing.

However, placebo had lower study discontinuation rates due to adverse events. Overall, APM may have antidepressant effects in the acute phase of MDD.

Summary: This study found that adjunctive L-theanine, when combined with sertraline, showed better efficacy in treating major depressive disorder (MDD) compared to placebo. The L-theanine group demonstrated significant improvement in depression symptoms and a greater reduction in depression scores compared to the placebo group at weeks 2, 4, and 6. Furthermore, the L-theanine group had higher remission and response rates at week 6. These findings suggest that L-theanine is a safe and effective adjunctive therapy for MDD.

Summary: Major depressive disorder is associated with increased risk of brain atrophy, aging-related diseases, and mortality. This study found that MDD patients showed a higher brain-predicted age difference compared to controls, but this difference did not seem to be driven by specific clinical characteristics. Subtle patterns of age-related structural brain abnormalities in MDD were observed, highlighting the need for further longitudinal studies to assess the clinical value of these findings.

Summary: Atypical antipsychotic (AAP) medication, in addition to treating psychiatric disorders, exhibits antimicrobial activity. However, long-term AAP use may lead to antimicrobial resistance in bacteria. This study found that exposure to the AAP quetiapine for 6 weeks resulted in multidrug antibiotic resistance in Escherichia coli. Mutations in the marR gene, which regulates the multiple antibiotic resistance (mar) operon, were identified as a potential mechanism for increased fitness of E. coli in the presence of quetiapine. These findings establish a link between the use of AAPs and the development of antimicrobial resistance in bacteria.

Summary: Depressive disorders, characterized by loss of interest in previously enjoyed activities and a depressed mood, are highly heterogeneous psychiatric diseases. Diagnosis and treatment of major depressive disorder (MDD) remain challenging due to significant clinical presentation heterogeneity and lack of applicable biomarkers. Identification of relevant biomarkers would enhance disease classification and personalized treatment strategies. In this review, we discuss the current state of these biomarkers and diagnostic techniques specifically targeting these analytes using state-of-the-art biosensor technology.

Summary: This study aims to identify intra-diagnostic subgroups of patients with bipolar disorder and assess the role of affective temperaments in aggressive behavior. Cluster analysis revealed three subgroups: non-aggressive, self-aggressive, and hetero-aggressive. Affective temperament scores significantly increased from non-aggressive to self-aggressive and hetero-aggressive groups. Predictive factors for subgroup membership included suicide attempts, rapid cycling, atypical antipsychotic or SNRI treatment, psychotic symptoms, and suicide attempts history.

Summary: This 3-month randomized psychoeducation-controlled trial examined the effects of exercise on gut microbiota in young adolescents with subthreshold depression. The exercise intervention group showed an increase in the relative abundance of certain genera and species of gut bacteria compared to the psychoeducation-controlled group. These changes were associated with improvements in depressive symptoms and enrichment of defense and signal transduction mechanisms.

Summary: This systematic review evaluates the efficacy, tolerability, and safety of oral ketamine, esketamine, and r-ketamine for unipolar and bipolar depression. The results indicate significant antidepressant effects and good tolerability of oral ketamine. However, the included randomized controlled trials had a high risk of bias, and further research with larger sample sizes and longer follow-up periods is needed to determine the antisuicidal effects and efficacy in treatment-resistant depression.

Summary: This study investigated the influence of age (early onset psychosis-EOP vs. adult onset psychosis-AOP) and diagnosis (schizophrenia spectrum disorders-SSD vs. bipolar disorders-BD) on the duration of untreated psychosis (DUP) and prodromal symptoms in patients with a first episode of psychosis. The results revealed that EOP patients had more prodromal symptoms and a longer DUP compared to AOP patients. Moreover, SSD patients had a significantly longer DUP than BD patients, and they also exhibited different profiles of prodromal symptoms. Understanding these differences can help improve the early detection of psychosis among minors.

Summary: This study examined the association between stressful life events (SLEs) and subthreshold depressive symptoms (SDS) and major depressive disorder (MDD), with a focus on gender effects. The findings showed that individuals who experienced any SLEs were at a higher risk of SDS and MDD. The association between SLEs and SDS was stronger in females, while the association between SLEs and MDD was stronger in males.

Summary: This study investigated the effects of liraglutide, a GLP-1 agonist, on a depression-like phenotype in mice exposed to chronic unpredictable stress. Liraglutide treatment reduced immobility in the forced swim test and improved cognitive function in the Morris water maze test. These results suggest that GLP-1 agonists could be potential novel antidepressants.

Summary: The present systematic review aimed to summarize the evidence about treatment-emergent mood switches in patients with bipolar disorder (BD). Thirty-two original studies met the inclusion criteria, and the majority focused on manic switches with limited research on depressive switches. Treatment-emergent mania/hypomania ranged from 17.3% to 48.8% and was more frequent with antidepressant monotherapy compared to combination treatment with mood stabilizers. Depressive switches were detected in 5-16% of patients and were associated with antipsychotic use. Methodological heterogeneity and small sample sizes were limitations in the included studies.

Summary: Despite evidence of widespread fractional anisotropy (FA) reductions in the brain white matter of bipolar disorder patients, there are still questions regarding the specificity and sensitivity of FA abnormalities compared to other diffusion metrics. A study using diffusion MRI was conducted on 316 participants, revealing abnormalities in other diffusion metrics in bipolar disorder, particularly in mean diffusivity (MD) and intra-axonal signal fraction (IASF). Machine Learning analysis showed that FA and IASF were the most helpful metrics for diagnosing bipolar disorder with an accuracy of 72%. Factors such as number of mood episodes, age of onset/duration of illness, psychotic symptoms, and current treatment with lithium and other medications were significantly associated with microstructure abnormalities, while lithium treatment was associated with fewer abnormal structures.

Summary: The study aimed to investigate the association between cannabis-related polygenic risk scores (PRS) and cannabis use and clinical course after first-episode psychosis (FEP). The results showed that cannabis use was associated with increased positive symptoms and younger age of cannabis initiation influenced the progression of symptoms. Patients with higher cannabis PRS had increased baseline cannabis use and the PRS for lifetime cannabis initiation was associated with negative and general symptomatology over follow-up. These findings suggest that genetic factors may play a role in cannabis use and symptom progression after FEP.

Summary: This review examines journal articles on clinical cases or vignettes of psychoanalysis or psychoanalytic psychotherapy for patients with bipolar disorder published from 1990 to 2021. The findings reveal that object relations was the most common theoretical approach adopted by the authors, and two main sets of clinical-theoretical considerations and recommendations emerged. The study suggests that further elaboration of the psychoanalytic literature on bipolar disorder can improve our understanding and provide valuable information for clinical and research purposes.

Summary: This study conducted a systematic review and meta-analysis to examine the KYN pathway metabolites in cerebrospinal fluid samples from patients with schizophrenia, bipolar disorder, and major depressive disorder. The results showed that KA levels were increased in schizophrenia, while the levels of TRP and KYN did not differ significantly. In bipolar disorder and major depressive disorder, there were no significant alterations in TRP and KA levels, although there may be a trend of increased KA levels and elevated QA levels in major depressive disorder. However, more studies are needed to confirm these findings.

Summary: This study examined the relationship among family accommodation (FA), posttraumatic stress symptoms (PTSS), and functional impairment. The results showed that FA was significantly associated with PTSS and functional impairment. Baseline FA partially mediated the relationship between baseline PTSS and functional impairment. Changes in FA from pre- to post-treatment were associated with relevant outcome variables at post-treatment and 12-month follow-up.
