Summary: The median delay in diagnosis was found to be 5.0 years in BD-I and 11.0 years in BD-II. Clinical factors such as earlier age of onset, lifetime suicide attempts, and comorbid anxiety disorders were associated with longer delay in diagnosis, while the presence of lifetime psychotic symptoms and psychiatric hospitalizations were associated with a shorter delay. In conclusion, the study highlights the need for strategies for early identification and interventions in Bipolar Disorder.

However, missing responses are common and there is little consensus on how to handle them. This study introduces the missing-response-incorporated log-signature method, achieving a correct diagnosis rate of 74.8% and outperforming other imputation approaches. The log-signature method is effective in analyzing prospectively collected mood data with missing responses, highlighting the clinical value of missing data.

Summary: Nitric oxide (NO) signaling has been studied in relation to mental illnesses, but its specific contribution remains unclear. This study investigated the association between peripheral NO concentration, specific diagnoses, and genetic variation in NO synthase (NOS) genes. The results showed that patients with schizophrenia had significantly higher peripheral NO metabolite concentrations compared to other groups. Additionally, carriers of the NOS1 VNTR1 short allele had increased NO concentrations, which remained significant even at discharge. The data also suggested that patients with unresolved depressive symptoms had higher NO concentrations, with a positive correlation between symptom severity and NO concentration.

This study established a novel ELISA method to analyze GluR4 levels in cerebrospinal fluid samples, and found that GluR4 may serve as a potential diagnostic biomarker for major depressive disorder. The findings contribute to the clinical application of GluR4 as a diagnostic biomarker for psychiatric disorders and enhance the understanding of their pathophysiological mechanisms.

Summary: This study examines whether the link between mothers diagnosed with bipolar disorder and emotional and behavioral problems in their offspring is mediated by disruptions in the offspring's biological rhythms. The findings show that offspring of mothers with bipolar disorder have more difficulty in maintaining biological rhythms and a higher prevalence of emotional and behavioral problems compared to offspring of mothers without bipolar disorder. Therefore, disruptions in the biological rhythms of the offspring fully mediate the impact of the mother's bipolar disorder on their emotional and behavioral problems.

Summary: This study retrospectively evaluated the applicability and progression of four staging models in a sample of 100 bipolar disorder patients. The results showed a pattern of stage worsening for each model, with a significant increase at each time point. Patients with lower education level, younger age at first episode, shorter illness duration, and shorter duration of untreated illness showed greater stage increases. Lower stage values were associated with certain characteristics, such as BD II, no psychiatric hospitalization, depressive onset, and few lifetime episodes.

Current literature suggests a relationship between compositional alterations in the gut microbiota and BD, with potential benefits in treating BD symptoms through modifications in the microbiota composition. Further research is needed to explore the potential of correcting gut microbiota alterations as a novel strategy in BD management.

The historical definitions of pseudo-hallucinations are reviewed, and six profiles are presented, considering manifestations, sequencing, and response to medication in bipolar disorder patients. The findings indicate the richness of expression, variable time course, and response to medication of pseudo-hallucinations in individuals with bipolar disorder.
