Pharmacogenomics of antidepressant induced mania: A review and meta-analysis of the serotonin transporter gene (5HTTLPR) association

Background: Antidepressants can trigger a rapid mood switch from depression to mania. Identifying genetic risk factors associated with antidepressant induced mania (AIM) may enable individualized treatment strategies for bipolar depression. This review and meta-analysis evaluates the evidence for association between the serotonin transporter gene promoter polymorphism (5HTTLPR) and AIM. Methods: Medline up to November 2009 was searched for key words bipolar, antidepressant, serotonin transporter, SLC6A4, switch, and mania. Results: Five studies have evaluated the SLC6A4 promoter polymorphism and AIM in adults (total N= 340 AIM+ cases, N = 543 AIM - controls). Although a random effects meta-analysis showed weak evidence of association of the S allele with AIM+ status, a test of heterogeneity indicated significant differences in estimated genetic effects between studies. A similar weak association was observed in a meta-analysis based on a subset of three studies that excluded patients on mood stabilizers; however the result was again not statistically significant. Limitations: Few pharmacogenomic studies of antidepressant treatment of bipolar disorder have been published. The completed studies were underpowered and often lacked important phenotypic information regarding potential confounders such as concurrent use of mood stabilizers or rapid cycling. Conclusions: There is insufficient published data to confirm an association between 5HTTLPR and antidepressant induced mania. Pharmacogenomic studies of antidepressant induced mania have high potential clinical impact provided future studies are of adequate sample size and include rigorously assessed patient characteristics (e.g. ancestry, rapid cycling, concurrent mood stabilization, and length of antidepressant exposure). The role of base excision repair in major depressive disorder and bipolar disorder

Summary: This systematic review examined the role of DNA repair enzymes, particularly base excision repair (BER), in mood disorders. The study found that SNPs of several BER genes are associated with MDD and BD, but conflicting findings and limited studies make it difficult to draw definitive conclusions about BER gene expression. Further research comparing DNA repair during episodes of mood disorders to remission may provide valuable insights into the role of DNA repair in these conditions.

Augmentation strategies for treatment resistant major depression: A systematic review and network meta-analysis

Summary: This study compared the efficacy and discontinuation of augmentation agents in adult patients with treatment-resistant depression through a systematic review and network meta-analyses. The findings suggest that certain adjunctive atypical antipsychotics, thyroid hormones, dopamine compounds, and lithium are superior in terms of efficacy. However, ziprasidone, mirtazapine, and cariprazine had higher discontinuation rates. Further research and head-to-head studies are needed to strengthen treatment options for treatment-resistant depression.

Summary: This study aimed to evaluate the clinical correlates and antipsychotic use patterns of tardive dyskinesia (TD+) in patients with bipolar disorder (BD). The prevalence rate of TD was 5.1%. TD+ participants were older, more likely to be female, and have type I bipolar illness compared to the TD- group. Limitations of the study included a cross-sectional design and the lack of severity assessment for tardive illness.

Summary: Bipolar disorder with co-occurring attention deficit-hyperactivity disorder (ADHD) is associated with a younger age of onset, male predominance, higher rates of comorbid anxiety and substance use disorders, and poorer response to mood stabilizing treatments. Genetic analysis shows that all subsets of bipolar disorder patients have higher genetic risk for both bipolar disorder and ADHD, with BD+cADHD patients having a higher ADHD genetic risk.

Summary: This systematic review found that women with BD have higher psychiatric and medical comorbidities compared to women without BD or other major psychiatric illness. Summary: This study investigated the relationship between mitochondrial function and the risk of treatment-emergent mania in patients with bipolar disorder. The results showed that antidepressants that increased mitochondrial energetics were associated with a higher risk of treatment-emergent mania. These findings suggest the need for a reclassification of antidepressants based on mitochondrial energetics and encourage future studies on specific antidepressants and mitochondrial activity.

Summary: Insulin resistance (IR) is associated with poorer clinical outcomes and inadequate treatment response in patients with bipolar disorder (BD). BD patients with IR exhibit impairments in verbal memory and executive function, as well as smaller hippocampal volumes and prefrontal neurochemical alterations. They are also more likely to have chronic and rapid cycling courses and lower Global Assessment of Functioning scores. BD patients with impaired glucose metabolism also have a higher rate of poor response to mood stabilizers.

The relationship between psychological pain and suicidality in patients with major depressive disorder: A meta-analysis

Summary: This study systematically reviewed the association between psychological pain and suicidality in patients with major depressive disorder (MDD). The results showed that psychological pain was a risk factor for suicidality in MDD patients, especially for those of advancing age. Reducing psychological pain in MDD patients is important for preventing suicidality.

The role of family accommodation in child posttraumatic stress symptoms and functional impairment in the context of cognitive behavioral therapy

Summary: This study examined the relationship among family accommodation (FA), posttraumatic stress symptoms (PTSS), and functional impairment. The results showed that FA was significantly associated with PTSS and functional impairment. Baseline FA partially mediated the relationship between baseline PTSS and functional impairment. Changes in FA from pre- to post-treatment were associated with relevant outcome variables at post-treatment and 12-month follow-up.
