Summary: Depressive episodes are common in bipolar disorder and often lead to poor functional outcomes. Limited treatments are approved for bipolar depression, but recent approval of cariprazine, a dopamine antagonist/partial agonist, shows it to be effective and well-tolerated.

Summary: This study reviewed the effectiveness and safety of pramipexole augmentation in the treatment of treatment-resistant depression. The results showed that pramipexole augmentation had a treatment response rate of 62.5% for both unipolar and bipolar depression, with good safety. However, further confirmation is needed.

Summary: The review and synthesis of existing literature on functional outcomes with ketamine/esketamine treatment in adults with treatment-resistant depression suggest mixed results across different functional measures, with improvements in general psychosocial functioning apparent. Future research is needed to explore the association between symptomatic improvement and functional improvement in TRD.

Summary: A retrospective analysis of 49 veterans with TRD receiving serial ketamine infusions showed that younger age was associated with faster clinical improvement in the early infusions, but age no longer predicted ketamine's antidepressant activity by infusion #6 and subsequent weeks. Age may mediate the speed but not the durability or total efficacy of ketamine treatment, suggesting dissociable mechanisms underlying different aspects of ketamine's antidepressant activity.

Summary: This study suggests that in a specialist inpatient setting, treatment difficulty can be predicted based on age and number of depressive episodes, but adding genetic risk scores does not improve prediction. Treatment is most effective for recurrent patients and those with a later age of onset, but patients with more severe resistance at admission remain a challenge.

Summary: This study evaluated the efficacy of acute intravenous ketamine treatment for treatment-resistant depression (TRD) and explored the peripheral blood-based biomarkers associated with treatment response. The results showed that ketamine significantly improved depressive symptoms and was well tolerated. In addition, baseline BDNF levels were found to be predictive of treatment response to ketamine.

Summary: This study aimed to identify expression biomarkers of treatment response for unipolar (UD) and bipolar (BD) depression through gene expression profiling in a naturalistic hospital setting. The results showed differentially-expressed genes (DEGs) in UD and BD patients after pharmacological treatment, indicating potential biomarkers associated with treatment response. Additional research focused on outlined genes may provide predictive markers for treatment outcomes in UD and BD.

Summary: Recent studies compare psychomotor reactivity disorders and gait activity in unipolar and bipolar depression, and find significant differences among the three groups. Bipolar patients exhibit more inhibited psychomotor skills than unipolar patients and healthy controls. The simplified variant of the equilibriometric task is the most sensitive and psychomotor reactivity is a more precise marker than psychomotor activity. These findings are important for distinguishing similar psychiatric conditions and developing new diagnostic and therapeutic approaches.

Summary: This study aimed to investigate differences in facial emotion recognition among patients with UD, BD, and normal controls. It found that UD patients performed poorly in recognizing negative expressions and had longer recognition times, while BD patients had lower accuracy in recognizing positive expressions and longer recognition times. Rapid facial expression recognition may serve as a potential endophenotype for early identification of unipolar and BD.

Summary: Sexual assault is associated with adverse psychological and physical health outcomes, including PTSD and depression. Neurobiological research has shown variations related to HPA axis, immune alterations, metabolic function, and brain circuitry in individuals with PTSD. The study found that women with PTSD after sexual assault had higher levels of ACTH and cortisol, which increased with symptom severity, especially in those with comorbid depression.

Summary: Evidence suggests that early life stress can disrupt the functioning of the HPA axis, leading to potential mental health disorders in adulthood. Dysregulation in hormone binding to receptors crucial for maintaining homeostasis may result in illness. The response of the HPA axis to stress plays a critical role in an individual's vulnerability to diseases related to psychological stress.

Summary: This study investigated the level of detection of depression by primary care clinicians and its determinants in low- to middle-income countries (LMICs). The results showed that the detection rate of depression was extremely low and significantly related to the severity of depression and suicidality. This poses a serious threat to scaling up mental healthcare in LMICs, highlighting the need for interventions to improve detection.
