Background: There is lack of data regarding BDNF serum levels in patients with obsessive-compulsive disorder (OCD). The aims of the present study were: 1) to assess the serum BDNF content in a sample of drug-naive patients with OCD and 2) to assess whether putative alterations in peripheral BDNF may be associated to OCD severity and clinical characteristics. Methods: Twenty-four drug-naive patients with a principal diagnosis of OCD were recruited. In parallel, a control group of 24 unrelated volunteers matched for gender and age was enrolled. Serum BDNF levels were measured by ELISA method. Results: The results showed that BDNF levels were decreased in OCD patients when compared to controls (36.90 +/- 6.42 ng/ml versus 41.59 +/- 7.82 ng/ml: p = 0.043). No correlations were evidenced between serum BDNF content and the severity of OCD symptoms measured as Y-BOCS scores or other clinical variables. Limitations: The choice of drug-naive patients with obsessive-compulsive disorder had limited the size of the sample and excluded the recruitment of patients with a severe symptomatology. Conclusions: Our findings reveal for the first time in OCD patients a decrease in serum BDNF levels. These data corroborate the hypothesis of a dysfunction in the neurotrophin expression in the OCD pathogenetic mechanism and provide the rationale for further investigations directed to the identification of novel biomarkers and new therapeutic strategies for antiobsessional treatments. Genome-wide association studies on Northern Italy isolated populations provide further support concerning genetic susceptibility for major depressive disorder

Vincenzo Dattilo, Sheila Ulivi, Alessandra Minelli, Martina La Bianca, Edoardo Giacopuzzi, Marco Bortolomasi, Stefano Bignotti, Massimo Gennarelli, Paolo Gasparini, Maria Pina Concas

Summary: This study identified new possible genes associated with major depressive disorder (MDD) using a genome-wide association study (GWAS) approach. The findings confirmed the polygenic nature of MDD. Further research is needed to better understand the role of these genes in MDD.

Language production impairments in patients with a first episode of psychosis

Giulia Gargano, Elisabetta Caletti, Cinzia Perlini, Nunzio Turtulici, Marcella Bellani, Carolina Bonivento, Marco Garzitto, Francesca Marzia Siri, Chiara Longo, Chiara Bonetto, Doriana Cristofalo, Paolo Scocco, Enrico Semrov, Antonio Preti, Lorenza Lazzarotto, Francesco Gardellin, Antonio Lasalvia, Mirella Ruggeri, Andrea Marini, Paolo Brambilla

Summary: This study investigates the characteristics of language difficulties and their relation with other cognitive domains in patients with a first episode of psychosis. The results show that patients with psychosis have language production deficits at both micro- and macro-linguistic levels, and these deficits are not related to other cognitive domains. Machine learning methods using language features are able to classify participants as patients or healthy controls with high accuracy.

Summary: This study aimed to compare the efficacy of esketamine in treating unipolar and bipolar treatment-resistant depression (TRD) and provide preliminary evidence of its effectiveness in bipolar TRD (B-TRD). The results showed that esketamine significantly reduced depressive symptoms in B-TRD patients compared to TRD patients, and it had a better anxiolytic action in B-TRD patients. The low risk of side effects confirmed the safety of this treatment.

Which patients benefit from adding short-term psychodynamic psychotherapy to antidepressants in the treatment of depression? A systematic review and meta-analysis of individual participant data

Summary: This study examined the efficacy moderators of combining short-term psychodynamic psychotherapy (STPP) with antidepressants for adults with depression. The results showed that adding STPP to antidepressants was more effective for patients with high baseline depression levels and longer depressive episode duration. These findings suggest that depression severity and episode duration should be considered when selecting treatment options for depression.

Assessing Adult ADHD Through Objective Neuropsychological Measures: A Critical Overview

Gianluca Rosso, Caterina Portaluppi, Elena Teobaldi, Gabriele Di Salvo, Giuseppe Maina

Summary: This review explores the role of neuropsychological tests in the diagnostic assessment of adult ADHD, specifically their ability to differentiate individuals with ADHD from those with other psychiatric conditions. The results indicate that among the objective measures analyzed, only Continuous Performance Tests, when combined with clinical interview instruments, were able to reliably distinguish individuals with ADHD from other psychiatric patients. Other objective tests showed mixed and inconsistent results. These findings highlight the need for further research to develop objective measures tailored to the core symptoms of ADHD, in order to improve the tests' diagnostic ability and assist clinicians in accurately distinguishing between ADHD and other psychiatric disorders.

Summary: This study used machine learning techniques to investigate sociodemographic and clinical features predicting responses to ESK-NS in TRD patients. The study found that features like severe anhedonia, anxious distress, mixed symptoms, and bipolarity were positively predictive of response and remission. However, benzodiazepine usage and depression severity were linked to delayed responses.

Risk factors for mortality after hospitalization for suicide attempt: results of 11-year follow-up study in Piedmont Region, Italy

Emina Mehanovic, Gianluca Rosso, Gian Luca Cuomo, Roberto Diecidue, Giuseppe Maina, Giuseppe Costa, Federica Vigna-Taglianti

Summary: The purpose of this study is to investigate the risk factors and temporal span for mortality in suicide attempters. The study found that the highest mortality occurred within the first 12 months after the attempt and remained elevated for many years afterwards. Male gender, older age, high deprivation index, single-parent family, mood disorders, malignant neoplasms, diabetes mellitus, and intracranial injuries were identified as independent predictors of death. The findings highlight the need to design strategies for long-term assistance and care to reduce unfavorable outcomes.

The pharmacological bases for repurposing statins in depression: a review of mechanistic studies

Summary: This review aims to gather available evidence from mechanistic studies to strengthen the pharmacological basis for repurposing statins in depression. The selected studies described several mechanisms potentially contributing to the effects of statins on mood, including serotonergic neurotransmission, neurogenesis, neuroplasticity, hypothalamic-pituitary axis regulation, and inflammation modulation. However, the association between statins, lipid metabolism, and worsening of depressive symptoms remains controversial.

Paliperidone Palmitate: A Breakthrough Treatment for Schizophrenia? A Review on Patient Adherence Levels, Healthcare Resource Utilization and Costs

Stefano Bramante, Gabriele Di Salvo, Giuseppe Maina, Gianluca Rosso

Summary: International guidelines recommend long-term antipsychotic therapy for schizophrenia, but medication compliance remains a critical issue. Paliperidone palmitate is an approved second-generation antipsychotic long-acting injectable for schizophrenia treatment. This systematic review aimed to evaluate PP-LAI adherence levels and healthcare resource utilization and costs. The findings suggest that paliperidone palmitate is a good treatment strategy for schizophrenia patients, with better adherence patterns and more favorable healthcare resource utilization and costs compared to oral antipsychotics.
