Summary: Obsessive-compulsive disorder (OCD) is characterized by obsessive thoughts and compulsive behaviors. The interactions and causal associations between these two behaviors are still unclear. To fill this gap, we combine a cortico-basal ganglia-thalamic model of motor loops and a limbic system model based on reward prediction errors to understand the complex dynamics of OCD. Our results suggest that reinforcement learning affects synaptic gain and loop stability, contributing to the imbalance between loops and the compulsive actions. Learning bridges the gap between obsession and compulsion, facilitating the transmission and progression of OCD. Our model provides theoretical explanations for the behavioral features of OCD and may suggest new treatment approaches.

Summary: Obsessive-compulsive symptoms (OCS) commonly occur in patients treated with clozapine and can significantly affect subjective wellbeing. This study found that OCS severity had a negative impact on wellbeing scores, independent of depressive and psychotic symptoms. However, OCS did not impair general functioning. Obsessional thinking and hoarding behavior, rather than compulsions, were specifically associated with decreased wellbeing, possibly due to the nature of the compulsions.
