Summary: We report a rare case of simultaneous pleomorphic adenoma in the parotid gland and parapharyngeal space in a 65-year-old male. The patient presented with a left parotid lump and examination revealed the displacement of the left palatine tonsil. CT scan showed a separate lump in the left parapharyngeal space and FNAC suggested the presence of mucoepidermoid carcinoma. The superficial parotid lump was excised first, followed by the excision of the parapharyngeal growth. Histopathology confirmed both growths to be pleomorphic adenomas. Awareness and optimal investigation of synchronous salivary gland tumors is crucial for complete surgical excision and appropriate management.

Summary: This study found that semiquantitative magnetic resonance imaging parameters have high value in distinguishing pleomorphic adenoma (PA) from Warthin tumor (WT), especially in the early and delayed phases. T1-max TSc-CR in the early phase showed high sensitivity, specificity, and area under the curve at a cutoff value of 1.89, while T1-max TSc-CR in the delayed phase performed better at a cutoff value of 2.33. The combination of all semiquantitative parameters further improved the diagnostic efficiency.

Summary: This study aimed to differentiate neurogenic tumors and pleomorphic adenomas in the parapharyngeal space using the texture characteristics of MRI-T2WI. The texture features of the region of interest were analyzed using PyRadiomics, and statistically significant differences were found in the selected texture features between the two groups. The diagnostic efficiency was evaluated using ROC curves.

Summary: This study aimed to identify predictive factors for malignant transformation (MT) in recurrent parotid pleomorphic adenomas (PAs) based on preoperative clinical parameters. Patients with a clinical diagnosis of recurrent parotid PA were included retrospectively. Univariate and multivariate analyses were used to assess the association between clinicopathologic variables and MT occurrence. MT occurred in 11.8% of the 467 patients. Univariate analysis showed that three or more previous recurrences, newly developed facial nerve paralysis, difficulty in mouth opening, tumors with a diameter >= 2.4 cm, and abnormal neck lymph node enlargement were associated with MT. Multivariate analysis revealed that three or more previous recurrences, newly developed facial nerve paralysis, difficulty in mouth opening, and abnormal neck lymph node enlargement independently predicted MT occurrence.

Summary: This study analysed the outcomes of surgery for pleomorphic adenoma of the parapharyngeal space in relation to the surgical approach. The study included 21 patients, with the most common surgical approach being transparotid-transcervical approach, followed by transoral robotic surgery and conventional transoral surgery. Post-operative complications were minimal and no recurrence occurred.
