A Systematic Review of Crohn's Disease Case Definitions in Administrative or Claims Databases

Susan Hutfless, Ryan A. Jasper, Aman Tilak, Tamoghna Ghosh, Saurabh Kedia, Simon Liu, Nathalie H. Urrunaga, Matthew Josephson, Arshit Narang, Steve Miller, Po-Hung Chen, Shelly Joseph, Steven R. Brant

Summary: This systematic review examines case definitions of Crohn's disease and finds that more complex definitions involving multiple diagnostic codes and prescriptions have desirable diagnostic accuracy properties. The study suggests that health services researchers should validate case definitions in their cohorts and, if validation is not possible, use more complex definitions. Sensitivity analyses should be performed for studies without validated case definitions to confirm the robustness of their results.

Pre-existing Thyroid Autoimmunity and Risk of Papillary Thyroid Cancer: A Nested Case-Control Study of US Active-Duty Personnel

Donald S. A. McLeod, Sheryl A. Bedno, David S. Cooper, Susan M. Hutfless, Silvia Ippolito, Susan J. Jordan, Peter G. Matos, Rachel E. Neale, Elena Sabini, David C. Whiteman, Paul W. Ladenson, Patrizio Caturegli

Summary: This case-control study conducted within the US active-duty personnel cohort found an association between longstanding prior thyroid autoimmunity and the risk of papillary thyroid cancer. The association was not mediated by the diagnosis of thyroid autoimmunity and thyroid cancers were often diagnosed at an early stage when autoimmunity had been identified.

Summary: This study analyzed the effect of vitamin D supplementation on infection risk and found that vitamin D may reduce the number of antibiotic prescriptions, especially in individuals with low vitamin D levels.

Summary: Supplementing high-dose vitamin D may not significantly reduce the incidence of cataract surgery, regardless of age, sex, body mass index, serum vitamin D concentration, or ambient ultraviolet radiation.

Risk factors for infections after endoscopic retrograde cholangiopancreatography (ERCP): a retrospective cohort analysis of US Medicare Fee-For-Service claims, 2015-2021

Susan Hutfless, Yasutoshi Shiratori, Daniel Chu, Simon Liu, Anthony Kalloo

Summary: The study found that urgent ERCPs, patient chronic conditions, and patient demographics are risk factors for post-ERCP infection. The use of disposable duodenoscopes can also reduce the risk of infection.

Vitamin D supplementation and hospitalization for infection in older adults: A post-hoc analysis of data from the Australian D-Health Trial

Summary: Recent studies have suggested that vitamin D supplementation may reduce the severity of infections, but its effect on hospitalization for infection has not been confirmed. This study found that vitamin D supplementation had no effect on the incidence of hospitalization for any infection, but it reduced the number of extended hospitalizations lasting more than 6 days.

The effect of vitamin D supplementation on pain: an analysis of data from the D-Health randomised controlled trial

Summary: Observational studies suggest an inverse association between 25-hydroxy vitamin D (25(OH)D) concentration and pain, but intervention trials have inconsistent findings. In this large placebo-controlled trial, vitamin D supplementation with 60000 IU of vitamin D-3/month had negligible effect on bodily pain.

Effect of vitamin D supplementation on depression in older Australian adults

McLeod, Gunter Hartel, Rachel L. O'Connell, Hai Pham, James G. Scott, Jolieke C. van der Pols, Alison J. Venn, Penelope M. Webb, David C. Whiteman, Rachel E. Neale

Summary: The study investigated whether vitamin D supplementation reduces depressive symptoms and incidence of antidepressant use. The results showed that high-dose monthly supplementation with vitamin D-3 did not have overall benefit for measures of depression, but there was some evidence of benefit in subgroup analyses.

Transthyretin binds soluble endoglin and increases its uptake by hepatocytes: A possible role for transthyretin in preeclampsia?

Melanie Young, Ming Tang, Huika Li, Derek J. Richard, Donald S. A. McLeod, Michael C. d'Emden, Kerry Richard

Summary: Preeclampsia is a life-threatening condition caused by poor placentation during pregnancy. The study suggests that transthyretin, a thyroid hormone binding protein, binds to soluble endoglin (sEng) and enhances its uptake by hepatocytes. This finding may lead to potential treatment options for reducing sEng-induced vascular dysfunction.

Thyroid cancer

Debbie W. Chen, Brian H. H. Lang, Donald S. A. McLeod, Kate Newbold, Megan R. Haymart

Summary: The past 5-10 years have seen significant advancements in the diagnosis and management of thyroid cancer, including ultrasound-based risk stratification systems, less invasive alternatives to surgery, and the availability of new systemic therapies for advanced cases. However, disparities in thyroid cancer care still exist, highlighting the importance of population-based studies and inclusive research to address barriers to equitable care.

The effect of monthly vitamin D supplementation on fractures: a tertiary outcome from the population-based, double-blind, randomised, placebo-controlled D-Health trial

Summary: This study investigated whether supplementing Australian adults with monthly doses of 60,000 IU vitamin D3 for up to 5 years altered the rate of fractures. The results showed that vitamin D supplementation did not increase fracture risk, and long-term supplementation might reduce the incidence of total fractures. However, further research is needed to confirm this effect.

Claims analysis of Medicare fee-for-service oral health care encounters, from 2019 through 2021

Darien J. Weatherspoon, Susan Hutfless, Richard J. Manski

Summary: A total of 2,098,056 oral health care encounters were identified through Medicare FFS claims during the study observation period, with a lower volume observed after 2019. Nearly 98% of encounters were related to those in which oral health diagnoses were recorded, and non-oral health care providers primarily submitted these claims. Most encounters included beneficiaries with chronic conditions, and a roughly equal proportion included those qualifying for Medicare on the basis of age and disability.

Vitamin D and Sun Exposure: A Community Survey in Australia

Vu Tran, Monika Janda, Robyn M. M. Lucas, Donald S. A. McLeod, Bridie S. S. Thompson, Mary Waterhouse, David C. C. Whiteman, Rachel E. E. Neale

Summary: Sun exposure in Australia has both negative and positive effects. It is a major cause of skin cancer, but also promotes vitamin D production. However, despite high levels of UV radiation, a significant portion of the population is vitamin D deficient. Balancing the risks and benefits requires effective communication.
