Summary: Non-invasive ventilation is an effective strategy for managing acute respiratory failure, but its role in acute hypoxemic respiratory failure is still debated. Helmet support, delivering high positive end-expiratory pressure, has been suggested to have potential advantages in terms of clinical outcome compared to other non-invasive strategies in some studies, but more research is needed for confirmation.

Brain monitoring after cardiac arrest

Summary: The purpose of this review is to summarize the available neuromonitoring tools for comatose patients after resuscitation from hypoxic-ischemic brain injury (HIBI). Recent findings suggest that EEG is useful for detecting seizures and identifying irreversible HIBI. Cerebral blood flow (CBF) alterations, as well as near-infrared spectroscopy (NIRS), intracranial pressure (ICP), transcranial Doppler (TCD), and brain biomarkers, are being investigated to optimize CBF and assess the severity and prognosis of HIBI. Neuromonitoring plays a crucial role in detecting complications, personalizing treatment, and predicting outcomes in HIBI patients.

Long-term clinical evolution of patients with prolonged disorders of consciousness due to severe anoxic brain injury: A meta-analytic study

Summary: This study assessed the long-term outcome and predictive value of demographic and clinical information for patients with prolonged disorders of consciousness caused by anoxic brain injury. The results showed that the mortality rate was 26%, the rate of any clinical improvement was 26%, and the rate of full recovery of consciousness was 17%. Younger age, a baseline diagnosis of minimally conscious state, higher Coma Recovery Scale Revised total score, and earlier admission to intensive rehabilitation units were associated with a higher likelihood of survival, clinical improvement, and recovery of full consciousness.

Summary: The study aimed to assess the effects of a protective ventilation strategy during Trendelenburg pneumoperitoneum surgery on postoperative oxygenation. A randomized trial was conducted with patients randomized into standard ventilation group and protective ventilation group during anesthesia. The results showed that there was no significant difference in PaO2/FiO2 one hour after extubation between the protective ventilation group and the standard ventilation group, but the protective ventilation group exhibited less injurious ventilation during the surgery.

Summary: Sepsis is a major cause of disability and death worldwide. The abnormal host response to pathogens results in unbalanced organ crosstalk and multi-organ dysfunction, which is directly associated with mortality. Etiologic interventions, such as antimicrobial therapy and control of the source infection, have been shown to improve outcomes. However, there are concerns about suboptimal antimicrobial exposure in critically ill septic patients receiving extracorporeal organ support. Strategies have been implemented to address this issue. This review discusses the pharmacokinetic features of antimicrobials and mechanisms that may affect drug removal during various forms of extracorporeal organ support.

The use of digital tools and virtual reality interventions is becoming increasingly important in ICU settings. Psychologists play an essential role in providing psychological support in ICUs.

Summary: This study assessed the 6-month outcomes of COVID-19 hypoxemic respiratory failure patients treated with helmet noninvasive ventilation (NIV) or high-flow nasal oxygen. The results showed similar quality of life and functional outcomes for both treatment methods. Worse outcomes were associated with the need for invasive mechanical ventilation. These data suggest that helmet NIV can be safely used in hypoxemic patients.

Can carotid artery Doppler variations induced by the end-expiratory occlusion maneuver predict fluid responsiveness in septic shock patients?

Summary: This study investigates the feasibility of using carotid Doppler (CD) to assess fluid responsiveness in patients with septic shock. The changes in CD peak velocity (CDPV) were found to be correlated with cardiac index (CI) changes and could predict fluid responsiveness. However, the changes in CDPV and corrected flow time (cFT) were not accurate predictors of fluid responsiveness.
