Context Fluoroquinolones are commonly prescribed classes of antibiotics. Despite numerous case reports of ocular toxicity, a pharmacoepidemiological study of their ocular safety, particularly retinal detachment, has not been performed. Objective To examine the association between use of oral fluoroquinolones and the risk of developing a retinal detachment. Design, Setting, and Patients Nested case-control study of a cohort of patients in British Columbia, Canada, who had visited an ophthalmologist between January 2000 and December 2007. Retinal detachment cases were defined as a procedure code for retinal repair surgery within 14 days of a physician service code. Ten controls were selected for each case using risk-set sampling, matching on age and the month and year of cohort entry. Main Outcome Measure The association between retinal detachment and current, recent, or past use of an oral fluoroquinolone. Results From a cohort of 989 591 patients, 4384 cases of retinal detachment and 43 840 controls were identified. Current use of fluoroquinolones was associated with a higher risk of developing a retinal detachment (3.3% of cases vs 0.6% of controls; adjusted rate ratio [ARR], 4.50 [95% CI, 3.56-5.70]). Neither recent use (0.3% of cases vs 0.2% of controls; ARR, 0.92 [95% CI, 0.45-1.87]) nor past use (6.6% of cases vs 6.1% of controls; ARR, 1.03 [95% CI, 0.89-1.19]) was associated with a retinal detachment. The absolute increase in the risk of a retinal detachment was 4 per 10 000 person-years (number needed to harm=2500 computed for any use of fluoroquinolones). There was no evidence of an association between development of a retinal detachment and beta-lactam antibiotics (ARR, 0.74 [95% CI, 0.35-1.57]) or short-acting beta-agonists (ARR, 0.95 [95% CI, 0.68-1.33]). Conclusion Patients taking oral fluoroquinolones were at a higher risk of developing a retinal detachment compared with nonusers, although the absolute risk for this condition was small. Summary: The study aimed to develop a risk prediction system for HF hospitalization in ACHD patients and successfully identified high-risk patients using predictors such as age, sex, CHD lesion severity, recent HFH history, and comorbidities. The risk scoring system showed excellent prediction performance.

Emulating a Randomised Controlled Trial With Observational Data: An Introduction to the Target Trial Framework

Stephen A. Kutcher, James M. Brophy, Hailey R. Banack, Jay S. Kaufman, Michelle Samuel

Summary: Randomised controlled trials (RCTs) are considered the highest quality of evidence, but observational studies can also mimic their effects. Using the target trial framework, observational studies can successfully emulate RCTs by focusing on key design elements.

Domperidone increases harmful cardiac events in Parkinson's disease: A Bayesian re-analysis of an observational study

Gisele Nakhle, James M. Brophy, Christel Renoux, Paul Khairy, Patrick Belisle, Jacques LeLorier

Summary: In assessing the risks of ventricular tachyarrhythmia/sudden cardiac death (VT/SCD) with domperidone use in Parkinson's disease (PD), Bayesian analysis with informative priors showed a high probability of increased risk that was robust to multiple prior sensitivity analyses. The frequentist and Bayesian approaches with an uninformative prior were unable to reach a definitive conclusion concerning the arrhythmic risk of domperidone in PD patients.

Summary: Most prediction models for hospitalization for heart failure in patients with type 2 diabetes have potential concerns with risk of bias or applicability, and uncertain external validity and clinical impact. Future research is needed to address these knowledge gaps.

Treatment and prescribing trends of antihypertensive drugs in 2.7 million UK primary care patients over 31 years: a population-based cohort study

Julie Rouette, Emily G. McDonald, Tibor Schuster, James M. Brophy, Laurent Azoulay

Summary: This study describes the prescribing trends of antihypertensive drugs in primary care patients and assesses the treatment trajectory of patients with hypertension according to changes in the UK hypertension management guidelines. The findings suggest that most patients are prescribed guideline-recommended first-line agents, but not all patients receive recommended drugs, particularly females, potentially leading to suboptimal cardiovascular outcomes.

Developing a cross-validation tool for evaluating economic evidence in rapid literature reviews

Xuanqian Xie, Chunmei Li, Sean Tiggelaar, Frances Simbulan, Lindsey Falk, James M. Brophy

Summary: This study developed a rapid cross-validation screening tool to evaluate the quality and credibility of economic evidence. The tool uses a qualitative approach under a Bayesian framework, utilizing prior health economic evidence for assessment. Practical considerations and an application of the tool are described in this article.

Dihydropyridine Calcium Channel Blockers and Risk of Pancreatic Cancer: A Population-Based Cohort Study

Julie Rouette, Emily G. G. McDonald, Tibor Schuster, James M. M. Brophy, Laurent Azoulay

Summary: In this population-based cohort study, new users of dihydropyridine calcium channel blockers (dCCBs) were not found to have an increased risk of pancreatic cancer compared to new users of thiazide diuretics after a median follow-up of 4.5 years. These findings provide reassurance regarding the long-term safety of these drugs in relation to pancreatic cancer risk.

Beta-Blockers, Digoxin, or Both Following an Incident Diagnosis of Atrial Fibrillation: A Prospective Cohort Study

James M. Brophy, Lyne Nadeau

Summary: This retrospective cohort claims database study explores the optimal drug choice for rate control in atrial fibrillation patients. The study finds that discharge prescriptions for digoxin alone or a combination of digoxin and beta-blockers are not associated with increased risk of recurrent cardiovascular hospitalizations and death compared to prescriptions for beta-blockers alone. More research is needed to improve the precision of these estimates.

Marginal structural models with latent class growth analysis of treatment trajectories: Statins for primary prevention among older adults

Awa Diop, Caroline Sirois, Jason Robert Guertin, Mireille E. Schnitzer, Bernard Candas, Benoit Cossette, Paul Poirier, James Brophy, Miceline Mesidor, Claudia Blais, Denis Hamel, Mina Tadrous, Lisa Lix, Denis Talbot

Summary: Latent class growth analysis combined with marginal structural modeling is proposed to classify individuals based on medication adherence pattern and estimate the effect of statin-usage trajectories on a first cardiovascular event. Simulation studies and analysis of real data demonstrate that our approach yields estimators with little or no bias.
