Antiviral Activity, Pharmacokinetics, and Safety of the HIV-1 Protease Inhibitor TMC310911, Coadministered With Ritonavir, in Treatment-Naive HIV-1-Infected Patients

Objectives: TMC310911 is a novel HIV type-1 (HIV-1) protease inhibitor with broad in vitro antiviral activity. In this phase 2a, open-label randomized study, the antiviral activity, pharmacokinetics, and safety and tolerability of ritonavir-boosted TMC310911 was assessed. Methods: In this study, treatment-naive HIV-1 patients (aged 18-60 years) received 1 of the 4 dosing regimens of TMC310911: 150 mg twice-daily (bid) (n = 8), 300 mg bid (n = 8), 75 mg bid (n = 9), or 300 mg once-daily (qd) (n = 8), for 14 days, all coadministered with 100 mg of ritonavir, as only antiretroviral therapy. Results: The mean change from baseline in HIV-1 RNA (log(10) copies per milliliter; primary efficacy endpoint) was -1.30 (75 mg bid), -1.14 (150 mg bid), -1.07 (300 mg bid), and -1.06 (300 mg qd) on day 8 and -1.53 (75 mg bid), -1.79 (150 mg bid), -1.69 (300 mg bid), and -1.55 (300 mg qd) on day 15. At steady state (day 14), the mean maximum plasma concentration and mean area under the plasma concentration-time curve from 0 to 12 hours tended to increase dose proportionally for bid doses; TMC310911 daily exposures for the 300 mg qd treatment and 150 mg bid treatment were comparable. The most common (>= 10%) treatment-emergent adverse events were fatigue (27.3%) and nausea (12.1%); no deaths or serious treatment-emergent adverse events were reported in this study. Conclusions: Combination treatment with TMC310911 and ritonavir showed potent antiviral activity (>1.5 log(10) copies/mL decrease in plasma HIV-1 RNA) at all evaluated doses, and treatment was generally safe and well tolerated.

Monkeypox in-patients with severe anal pain

Summary: This study reports the clinical characteristics of the first confirmed monkeypox patients admitted to a center in Berlin. Among the patients, most were MSM who had engaged in condomless receptive anal intercourse prior to admission. The main complaint was severe anal pain, and investigations revealed proctitis and anal and rectal ulcers with detection of the monkeypox virus. These findings suggest that sexual transmission may play a role in the current outbreak.

Summary: This study aimed to examine the incidence rates and predictive utility of established prognostic factors for the progression to AIDS among people living with HIV under clinical care. The study used data from two observational cohorts of people living with HIV in Germany and found that the rates of a first AIDS event have continuously declined, and health outcomes depend on CD4 count, viral load, and age but not on transmission mode.

Summary: Life expectancy has increased for people with HIV on antiretroviral therapy (ART). Latest prognosis data show that the survival rate of adults with HIV who have been on ART for at least 1 year in Europe and North America is similar to that of the general population, regardless of the starting time of ART. However, individuals with low CD4 counts at the start of treatment have lower life expectancy, highlighting the importance of early diagnosis and sustained treatment of HIV.

Summary: The objective of this study was to identify the pathogen spectrum of community acquired pneumonia in people living with HIV (PLWH), and to compare it with a matched HIV negative group in order to reassess therapeutic strategies for PLWH. The study used various methods to identify pathogens, and found that pneumococci were the most common pathogen among both PLWH and controls. The mortality rate was higher for PLWH compared to controls.

Summary: The clinical relevance of the mismatch between sputum smear microscopy and culture in patients with open pulmonary tuberculosis (opTB) was investigated in this study. The results show that a substantial number of patients who were de-isolated after 3 negative sputum smear microscopy tests still had positive culture results. Further investigation is needed to determine the infectivity of smear-negative and culturally-positive individuals on therapy.

Motile microfilaria captured by fluorescent microscopy and the unmasking of eosinophilia following treatment

Summary: A 24-year-old patient from Cameroon presented to our hospital with a foreign structure in her left eye. For the first time, fluorescent microscopy revealed motile microfilariae, and the diagnosis of loiasis was confirmed. Despite high levels of microfilaremia, eosinophilia only became evident after antiparasitic therapy was initiated.

Summary: Cancer incidence rates remained stable overall from 2006 to 2021, but there were changes in the rates of specific types of cancer among people with HIV. AIDS-defining cancers and infection-related cancers decreased, while non-AIDS-defining cancers, smoking-related cancers, and BMI-related cancers increased. These findings emphasize the importance of prevention strategies for reducing the incidence of non-AIDS-defining cancers, smoking-related cancers, and BMI-related cancers.
