Magnesium supplementation helps to improve carotid intima media thickness in patients on hemodialysis

The atherosclerotic process progresses more dynamically in hemodialysis (HD) patients than in the general population. In HD patients, lower magnesium levels were reported to be associated with increased atherosclerosis of the common carotid artery. We tested the hypotheses that magnesium supplementation helps to improve carotid intima media thickness (IMT) in HD patients. A total of 47 patients on HD were included in the study. Patients were randomly divided into two groups: group A (Mg group), in which patients were given magnesium citrate orally at a dosage of 610 mg every other day for 2 months and group B (control group), in which patients received only calcium acetate therapy as a phosphate binder. At baseline and 2 months later, all patients underwent a carotid artery ultrasound scan to measure carotid IMT. At the end of 2 months, mean serum calcium, phosphorus, and calcium x phosphorus product were not changed in both groups. As expected, mean serum Mg level significantly increased in the Mg group at the end of 2 months. In addition, serum parathyroid hormone (PTH) level significantly decreased in the Mg group at the end of 2 months (P = 0.003). Baseline carotid IMT was similar between the groups. Bilateral carotid IMT was significantly improved in patients treated with magnesium citrate compared to initial values (P = 0.001 for left, P = 0.002 for right). Based on the present data, magnesium may play an important protective role in the progression of atherosclerosis in patients on dialysis. Further studies are needed to assess more accurately the role of magnesium in atherosclerotic regression in dialysis patients.

Aligning research initiatives, strengthening electronic health records, and involving experts in study design and data analysis will be crucial in responding to the next pandemic.

Outcomes of COVID-19 in peritoneal dialysis patients: A report by the European Renal Association COVID-19 Database

Acute Kidney Injury: Medical Causes and Pathogenesis

Summary: Acute kidney injury (AKI) is a common clinical syndrome characterized by a sudden decline in or loss of kidney function. It is associated with substantial morbidity, mortality, and increased risk of chronic kidney disease (CKD). The classification of AKI into prerenal, intrarenal, and postrenal categories may be useful for differential diagnosis, but AKI is mostly multifactorial, with various pathophysiologic features. Acute tubular necrosis, caused by ischemia or nephrotoxicity, is common in AKI. Identifying AKI in a timely manner and understanding the mechanisms behind kidney dysfunction are crucial. This review discusses the medical causes and recent updates in the pathogenesis of AKI.

Effect of Hemodiafiltration or Hemodialysis on Mortality in Kidney Failure

Gut Microbiota in Chronic Kidney Disease: From Composition to Modulation towards Better Outcomes-A Systematic Review

Summary: This study systematically reviewed the gut microbiota composition in patients with chronic kidney disease (CKD). The results showed that CKD patients had decreased microbiota diversity and decreased abundance of Roseburia. Gut dysbiosis was also associated with peritonitis and enhanced inflammatory activity. Additionally, synbiotic and probiotic therapies were found to have a beneficial effect on gut flora composition.

Summary: Rates of late allograft loss have improved slowly in the last decades. Well described traditional risk factors that influence allograft survival include cardiovascular events, rejection, infections and post-transplant neoplasia. Here, we critically evaluate the influence of several non-immunological, non-traditional risk factors and describe their impact on allograft survival and cardiovascular health of kidney transplant recipients.

Thiol/disulfide homeostasis as a new oxidative stress marker in patients with Fabry disease

Summary: This study is the first to examine the thiol-disulfide balance and oxidative stress levels in patients with Fabry disease. The results show significant differences in thiol and disulfide levels between the patient and control groups, and these levels are correlated with lysosomal globotriaosylceramide, total oxidant status, and blood urea nitrogen. Additionally, the study found lower total antioxidant status and higher oxidative stress levels in Fabry disease patients.

Intrauterine life to adulthood: a potential risk factor for chronic kidney disease

Shared decision making in elderly patients with kidney failure

Summary: The definition of the elderly as individuals aged 65 years or older fails to consider the genetic, lifestyle, and overall health differences that contribute to significant heterogeneity among older adults. With the global aging population, the prevalence of chronic diseases, including chronic kidney disease (CKD), is increasing. Frailty is highly prevalent in this population and needs to be assessed to inform medical decisions. Choosing the optimal treatment pathway for elderly and frail patients with kidney failure is complex due to the presence of comorbidities associated with low survival rates and impaired quality of life. Multidisciplinary management involving healthcare professionals from various specialties is recommended. Current studies are mainly retrospective and observational, and lack adjustment for confounders or addressing biases, making it difficult to guide treatment decisions. Prospective studies and systematic reviews are needed to address these issues.

Efficacy and Safety of Ticagrelor versus Clopidogrel in Dialysis Patients with Coronary Syndromes: A Systematic Review and Meta-Analysis

Summary: This meta-analysis aimed to compare the efficacy and safety endpoints of ticagrelor and clopidogrel in end-stage kidney disease (ESKD) patients on chronic dialysis presenting with acute or chronic coronary syndromes (ACS or CCS). The results showed no significant difference in efficacy or safety outcomes between ticagrelor and clopidogrel. Further randomized controlled trials are needed to establish the optimal antiplatelet therapy in this population.

Plasma Apelin Levels and Thiol/Disulfide Balance in Patients with Type 2 Diabetes Mellitus

Summary: The study aimed to investigate the relationship between plasma apelin concentrations, oxidative stress biomarkers, and albuminuria in patients with type 2 diabetes mellitus. The results showed significantly decreased serum apelin concentrations and thiol levels in patients with type 2 diabetes. A negative correlation was found between serum apelin concentrations and glycemic control, as well as between thiol levels and albumin.

Clinical Implications and Risk Factors for Erectile Dysfunction in Kidney Transplant: A Single- Center Assessment

Summary: This study investigated the prevalence and impact of erectile dysfunction (ED) after kidney transplantation. The results showed that the prevalence of sexual dysfunction increased with age, and only a small percentage of patients had a normal sexual function. Additionally, the use of alpha-blockers and aspirin was associated with ED.
