Barriers to, and facilitators of post-operative pain management in Iranian nursing: a qualitative research study

Unrelieved post-operative pain continues to be a major clinical challenge, despite advances in management. Although nurses have embraced a crucial role in pain management, its extent is often limited in Iranian nursing practice. To determine Iranian nurses' perceptions of the barriers and facilitators influencing their management of post-operative pain. This study was qualitative with 26 participant nurses. Data were obtained through semi-structured serial interviews and analysed using the content analysis method. Several themes emerged to describe the factors that hindered or facilitated post-operative pain management. These were grouped into two main themes: (1) barriers to pain management after surgery with subgroups such as powerlessness, policies and rules of organization, physicians leading practice, time constraints, limited communication, interruption of activities relating to pain, and (2) factors that facilitated post-operative pain management that included the nurse-patient relationship, nurses' responsibility, the physician as a colleague, and nurses' knowledge and skills. Postoperative pain management in Iran is contextually complex, and may be controversial. Participants believed that in this context accurate pain management is difficult for nurses due to the barriers mentioned. Therefore, nurses make decisions and act as a patient comforter for pain after surgery because of the barriers to effective pain management.

Alteration of lumbar muscle morphology and composition in relation to low back pain: a systematic review and meta-analysis

Relationship between the morphology and composition of the lumbar paraspinal and psoas muscles and lumbar intervertebral motion in chronic low-back pain: An exploratory study

Summary: This study investigated the relationship between lumbar intervertebral motion and changes in lumbar morphology/composition in patients with chronic low-back pain. The results showed that patients with greater intervertebral motion had smaller cross-sectional areas of the multifidus muscle and larger cross-sectional areas of the lumbar erector spinae muscle. Muscle asymmetry was observed at different lumbar vertebral levels, with the greatest translational intervertebral motion occurring at the L3-L4 level and the greatest rotational translation at the L4-L5 level. Associations were found between the morphology of the paraspinal muscles at the adjacent vertebral levels and the increased intervertebral motion at the L3-L4 level. These findings provide a plausible biological reason for the effectiveness of rehabilitating deficient paraspinal muscles in a subset of patients with chronic low-back pain.

Meaning of Respect for Older People in Family Relationships

The results showed that respect for older people includes respect for their personal interests, kind and sincere respect, and respect for their autonomy. Family members should understand their expected roles in family relationships and consider respect as an important factor affecting older people's well-being.

Maternal serum adiponectin and leptin levels and their gene expression, postpartum body mass index, and breastfeeding duration based on mode of delivery: A prospective study

Summary: This study found differences in adiponectin and leptin levels, gene expression, body mass index, and breastfeeding duration between cesarean section and vaginal delivery. Cesarean section was associated with lower maternal leptin levels and shorter breastfeeding duration. However, there were no significant differences in adiponectin levels and gene expression between the two groups.

Combining clinical exams can better predict lumbar spine radiographic instability

Summary: This study evaluated the diagnostic accuracy of clinical lumbar instability tests and developed a model to detect lumbar instability. The results showed that each individual test had very limited ability to distinguish patients with lumbar instability, but the developed model slightly improved the accuracy of diagnosis.

The spousal role of middle-aged Iranian women: A qualitative content-analysis study

Summary: This study aimed to explain the spousal role in the lives of Iranian middle-aged women. Through semi-structured interviews and conventional content analysis of 25 middle-aged women, three main categories of paying more attention to the spouse's needs, enhancing feminine charms, and maintaining married life were identified as the theme of reviving the spousal role. This understanding could help healthcare personnel develop indigenous marriage-enrichment programs for middle-aged individuals.
