Summary: The clinical use of TMVR using transcatheter aortic valves is limited to patients with failing bioprostheses or rings, and mitral valve disease associated with severe mitral annulus calcification. While valve-in-valve TMVR seems to be a reasonable alternative to surgery for high-risk patients, there is less evidence supporting valve-in-ring and valve-in-mitral annulus calcification interventions.

Summary: The aim of the study was to provide real-world data on outcomes for elderly Taiwanese patients who underwent transcatheter aortic valve replacement or surgical aortic valve replacement in different risk groups. The results showed that in all risk groups, patients in the TAVI group had shorter hospital stay and higher rate of paravalvular leakage compared to the SAVR group, but there were no significant differences in mortality rates between the two groups. Risk factors for higher 1-year and 5-year mortality included BMI less than 20 and acute kidney injury.

The results showed similar clinical outcomes in terms of clinical endpoints in low- and medium-risk groups, but differences were observed in certain indicators such as paravalvular leak, acute kidney injury, and new onset atrial fibrillation.

Summary: This case report describes a rare complication of prosthetic valve detachment in Behcet's disease. A 30-year-old woman with a history of recurrent oral ulcers and cutaneous lesions underwent aortic valve replacement three months prior and experienced detachment and destruction of the intervalvular fibrous body. Reoperation and peri-operative steroid therapy were performed.

Summary: The study compared outcomes of kidney transplant-eligible patients on dialysis undergoing heart valve replacement procedures. Mechanical aortic valve replacement was associated with a significantly lower risk of death compared to bioprosthetic valve replacement, while there were no significant differences in outcomes for mitral valve replacement. The findings suggest that individualized decision-making is recommended for this special group of patients.

Summary: Low-flow low-gradient AS affects 10.5% of severe AS patients, with a lower need for AVR compared to the HG group and similar to the NFLG group, with no differences in mortality. AVR has a lower impact on LFLG AS compared with HG AS. The study shows LFLG AS to have an intermediate clinical risk profile between the HG and NFHG groups.

Summary: This study analyzed the management and mortality differences in the three iterations of the Spanish Registry of Acute Aortic Syndrome (RESA). The use of computed tomography as the first diagnostic technique increased over time, while surgical management and overall mortality decreased in type A cases. In type B cases, endovascular treatment increased and overall mortality decreased, particularly with medical and endovascular treatments.
