Loss of pons-to-hypothalamic white matter tracks in brainstem obesity

Hyperphagia and obesity have been reported following damage to the hypothalamus in humans. Other brain sites are also postulated to be involved in the control of food intake and body weight regulation, such as the amygdala and brainstem. The brainstem, however, is thought to primarily integrate short-term meal-related signals but not affect long-term alterations in body weight, which is controlled by higher centers. The objective of this study was to identify structural pathways damaged in a patient with a brainstem cavernoma who experienced sudden onset of hyperphagia and > 50 kg weight gain in <1 year following surgical drainage via a midline suboccipital craniotomy. Diffusion tensor imaging revealed loss of nerve fiber connections between her brainstem, hypothalamus and higher brain centers with preservation of motor tracks. Imaging and endocrine testing confirmed normal hypothalamic structure and function. Gastric bypass surgery restored normal appetite and body weight to baseline. This is the first report of 'brainstem obesity' and adds to the brain regions that can determine the long-term body weight set point in humans.

Weight gain trajectories and obesity rates in intensive and conventional treatments of type 1 diabetes from the DCCT compared with a control population without diabetes

Summary: Obesity is a significant health issue for participants with type 1 diabetes undergoing intensive diabetes management. The study compared weight gain in those receiving intensive and conventional type 1 diabetes treatment, as well as a population without diabetes. It was found that weight gain with intensive therapy occurred in two stages and resulted in similar or higher obesity rates compared to the population without diabetes after 2 years. Glucose control and family history were identified as influential factors.

Real-world utilization of bempedoic acid in an academic preventive cardiology practice

Summary: This study evaluated the real-world use of bempedoic acid for lipid management in patients with cardiovascular disease. The results showed that bempedoic acid was effective in reducing LDL-C levels in patients at high risk for atherosclerotic events and statin intolerance. However, high rates of treatment-emergent adverse events and drug discontinuations were observed.

The results showed that the off-protocol meal increased daily caloric intake without affecting appetite ratings. There was a slight reduction in intake the following day, but no change in appetite scores. Protein intake was inversely associated with daily caloric intake.
