Sex-dependent role of glucocorticoids and androgens in the pathophysiology of human obesity

Obesity, particularly its abdominal phenotype, a harbinger of the metabolic syndrome, cardiovascular diseases (CVDs) and type 2 diabetes mellitus (T2D), is becoming one of the most significant public health problems worldwide. Among many other potential factors, derangement of multiple hormone systems have increasingly been considered for their potential importance in the pathophysiology of obesity and the metabolic syndrome, with particular reference to glucocorticoids and sex hormones. These systems have a fundamental and coordinating role in the physiology of intermediate metabolism and cardiovascular function, and in the response to acute and chronic stress challenge. Abdominal obesity is associated with a hyperactivity of the hypothalamic-pituitary-adrenal (HPA) axis and impaired androgen balance, although these alterations differ according to sex. As there is also increasing evidence that there are many differences between the sexes in the susceptibility and development of obesity, T2D and CVDs, we support the hypothesis that alterations of the HPA axis and androgen balance may have an important function in this context. This is further supported by the fact that there are important differences between males and females in their ability to adapt to both internal and particularly to environmental (external) stressors. In addition, there is also evidence that, in both physiological and pathological conditions, a close cross talk exists between sex hormones and glucocorticoids at both neuroendocrine and peripheral level, again with different specificities according to sex.

Summary: Independent predictors of operative time include gender, symptoms, previous upper abdominal surgery, size of the lesion, and cumulative sum of procedures. When compared between different surgeons, operative time was significantly longer for the senior surgeon than for the two junior surgeons. The presence of a mentor in the operative theater and specific training programs may be useful during the learning period.

Summary: The study aimed to compare the sensitivity and specificity of different imaging techniques for preoperative localization in patients with primary hyperparathyroidism, as well as analyze the cost utility of each technique. The results showed that color Doppler US and 4D-CT have higher sensitivity in parathyroid localization, especially in patients without thyroid diseases.

Summary: Research on cardiovascular disease prevention has shown that obesity, rather than the hyperandrogenic phenotype, has a greater impact on ultrasound cardiovascular risk markers in PCOS. Epicardial fat thickness (EFT) showed the strongest association with BMI, highlighting its potential for estimating cardiovascular risk in PCOS.

Summary: This study aimed to evaluate the role of 18F-FDG PET/CT in the clinical management of RAI-R-DTC and confirmed its obviation of further unnecessary RAIT. Results showed that among 53 patients, approximately half had a positive 18F-FDG PET/CT scan, leading to different therapeutic approaches, with significant associations found between metabolic response and final disease response evaluation.

Summary: Follicular thyroid carcinoma (FTC) is the second most common malignant thyroid neoplasm after papillary carcinoma (PTC), with a tendency to metastasize to distant sites such as bone and lung. Understanding of the molecular pathology of thyroid tumors has greatly improved over the last 20 years, with the identification of uncommon BRAF non-V600E mutations believed to be associated with low malignant potential follicular patterned tumors.
