Mortality in ankylosing spondylitis: results from a nationwide population-based study

Objectives Information on mortality in ankylosing spondylitis (AS) is scarce. Our study therefore aimed to assess: (1) mortality in AS versus the general population, and (2) predictors of death in the AS population. Methods Nationwide cohorts of patients with AS diagnosed at rheumatology or internal medicine outpatient clinics (n=8600) and age-matched, sex-matched and county-matched general population comparators (n=40 460) were identified from the National Patient Register and the census register, respectively. The follow-up period began on 1 January 2006 or at the first date of registered diagnosis thereafter and extended until death, emigration or 31 December 2012, whichever occurred first. Socioeconomic variables, AS-related clinical manifestations, joint surgery, comorbidities and medication were identified from other national registers. Cox regression models were used to determine mortality and predictors for death in the AS cohort. Results There were 496 deaths in the AS cohort and 1533 deaths in the control cohort resulting in an age-adjusted and sex-adjusted HR of 1.60 (95% CI 1.44 to 1.77), with increased mortality for men (age-adjusted HR=1.53, 95% CI 1.36 to 1.72) and women (ageadjusted HR=1.83, 95% CI 1.50 to 2.22). Within the AS cohort, statistically significant predictors for death were a lower level of education, general comorbidities (diabetes, infections, cardiovascular, pulmonary and malignant diseases) and previous hip replacement surgery. Conclusions Mortality was increased for male and female patients with AS. Predictors of death within the AS cohort included socioeconomic status, general comorbidities and hip replacement surgery.

Summary: This study compares the effectiveness of secukinumab with tumor necrosis factor inhibitors (TNFi) in treating patients with spondyloarthritis (SpA), using adalimumab as the main comparator. The findings show that secukinumab has similar treatment retention as adalimumab in the first or second line of therapy, but poorer outcomes in the third-plus line. Secukinumab also has lower response rates than adalimumab, particularly in the third-plus line of treatment.

Summary: The purpose of this study was to investigate the changes in different cell types and disease-related factors in patients with ankylosing spondylitis (AS) from northern Sweden. The study found a significant reduction in the number of TFH cells, plasmablasts, and B memory cells in AS patients, which may be related to an aberrant humoral immune response associated with inflammation. Some cell types in female patients were negatively correlated with inflammation markers.

Summary: The objective of this study was to assess and compare the incidence rates of key safety outcomes for individual targeted synthetic or biological disease-modifying antirheumatic drugs (b/ts DMARDs) in rheumatoid arthritis (RA). The study found that the b/tsDMARDs currently used to treat RA have acceptable and largely similar safety profiles, but differences exist in particular concerning tolerability and specific infection risks.

The National Prevalence of Clinically Diagnosed Psoriatic Arthritis in Sweden in 2017

Summary: This study aimed to estimate the prevalence of clinically diagnosed Psoriatic Arthritis (PsA) in Sweden in 2017 and assess the use of disease-modifying antirheumatic drugs (DMARDs) among PsA patients in contact with specialized rheumatology care. The prevalence of PsA in Sweden was approximately 0.35% among adults aged 18 to 79 years in 2017, and nearly three-fourths of PsA patients in recent contact with specialized rheumatology care received DMARD therapy in 2017.

Methotrexate treatment in early psoriatic arthritis in comparison to rheumatoid arthritis: an observational nationwide study

Summary: This study aimed to compare the treatment outcomes of patients with newly diagnosed psoriatic arthritis (PsA) and rheumatoid arthritis (RA) using methotrexate (MTX) monotherapy. The results showed that PsA patients were less likely to continue the use of MTX compared to RA patients, and the effectiveness of MTX was also lower in PsA.

The ASAS Health Index and Environmental Factors Item Set: validity and reliability of the Swedish translations in Swedish patients with ankylosing spondylitis

Summary: The objective of this study was to translate the ASAS HI EFIS into Swedish and culturally adapt it for a Swedish context, and to assess the construct validity of the Swedish version of ASAS HI and test-retest reliability in ASAS HI and EFIS in Swedish patients with AS. The results showed that the Swedish version of ASAS HI is valid and reliable, and recommended for assessing the impact of AS on global functioning and health.

Association of childhood infections and perinatal factors with ankylosing spondylitis: a Swedish nationwide case-control and sibling study

Summary: This study aims to identify the risk factors for ankylosing spondylitis (AS) during perinatal and early life, while controlling for family-shared confounding using a sibling comparison design. The study found that having older siblings and a history of tonsillectomy in childhood were independently associated with the development of AS, even after adjusting for family-shared factors in a sibling comparison analysis.

Summary: This study assessed the dietary nutrient intake in patients with r-axSpA and compared it to persons without r-axSpA. The results showed that the patients had impaired dietary intake, particularly in nutrients with anti-inflammatory properties. Therefore, it is suggested to incorporate nutrition screening into the management of r-axSpA patients.

Physical function and sex differences in radiographic axial spondyloarthritis: a cross-sectional analysis on Bath Ankylosing Spondylitis Functional Index

Summary: This study examined the factors that influence BASFI scores in r-axSpA patients, both overall and stratified by sex. The results showed that age, body mass index, disease activity, fatigue, and tenderness were independently associated with BASFI. There were no significant differences in BASFI scores between males and females, but certain individual questions showed gender differences, such as worse ability to look over shoulder in males and worse ability to climb stairs in females.
