Suppression-Replacement KCNQ1 Gene Therapy for Type 1 Long QT Syndrome

Summary: This study presents the first proof-of-principle gene therapy for complete correction of long QT syndrome, where KCNQ1-SupRep successfully suppressed and replaced KCNQ1 to normal wild-type levels. In TSA201 cells, cotransfection of LQT1-causative variants and KCNQ1-SupRep achieved mutation-independent suppression and replacement of KCNQ1. In LQT1 iPSC-CMs, KCNQ1-SupRep gene therapy shortened the APD, eliminating the pathognomonic feature of LQT1.

Association of the P441L KCNQ1 variant with severity of long QT syndrome and risk of cardiac events

Summary: In this study, we report a missense mutation P441L in the C-terminus of KCNQ1 gene, which leads to a severe LQT1 phenotype in a 37-year-old female patient. Variant P441L shows reduced membrane trafficking and interaction with KCNQ1-KCNE1, resulting in potassium efflux disorder and eventually causing LQT1.

Divergent regulation of KCNQ1/E1 by targeted recruitment of protein kinase A to distinct sites on the channel complex

Summary: The study investigated the reconstitution of PKA regulation of I-Ks in the absence of AKAP9 using a targeted protein phosphorylation approach. The results showed that targeted recruitment of endogenous Ca to E1-YFP enhanced I-Ks while recruitment to Q1-YFP inhibited IKs. Proteomic analysis revealed different phosphorylation sites modified by Ca targeting Q1-YFP compared to free Ca. This study provides insights into the regulation of IKs by phosphorylation and suggests a potential approach to prevent exercise-induced sudden cardiac death.

Mechanistic understanding of KCNQ1 activating polyunsaturated fatty acid analogs

Summary: The KCNQ1 channel plays a vital role in the repolarization phase of the cardiac action potential. Mutations in KCNQ1 can lead to long QT syndrome (LQTS), causing arrhythmia and sudden cardiac death. Previous research has shown that polyunsaturated fatty acids (PUFAs) and PUFA analogs can activate the KCNQ1 channel, offering potential therapeutics for LQTS treatment. Different binding sites on KCNQ1 are preferred by PUFA analogs, with binding between S3 and S4 resulting in the greatest shift in voltage dependence. Understanding these interactions can aid in the development of drugs for long QT syndrome.

Summary: This study aimed to determine the characteristics and survival of patients transplanted for refractory electrical storm. The results showed an in-hospital mortality rate of 28.9% and a one-year survival rate of 68.9% in patients who received heart transplantation after electrical storm. Larger studies are needed to determine the predictors of in-hospital mortality in high-risk patients.
