Summary: This study aimed to collect clinical outcome data and identify risk factors for a complicated course of COVID-19 in patients with adult congenital heart disease (ACHD). Risk factors such as age, obesity, and multiple comorbidities were associated with an increased risk of a complicated COVID-19 course in ACHD patients. Cyanotic heart disease, including unrepaired cyanotic defects or Eisenmenger syndrome, were identified as particularly high-risk congenital cardiac defects in ACHD patients.

Summary: Between 1980 and 1997, 8 out of 10 individuals with congenital heart defects (CHDs) in the US survived until 35 years of age, with survival rates varying based on the severity of CHDs, presence of noncardiac anomalies, birth weight, and maternal race and ethnicity. Among individuals without noncardiac anomalies, those with nonsevere CHDs had similar mortality rates between 1 and 35 years of age as the general population, and those with any CHD had similar mortality rates between 10 and 35 years of age as the general population.

Summary: This study aimed to evaluate the long-term mortality and cause-specific mortality of patients with atrial septal defect (ASD) in a nationwide cohort. The results showed that patients with ASD had higher overall mortality, with increased cause-specific mortality in congenital malformations, stroke, and heart diseases.

Summary: This study aimed to analyze the outcomes of adults over the age of 40 with complete atrioventricular septal defects (cAVSD), finding significant morbidity and mortality in this patient population. The New York Heart Association (NYHA) class at the beginning of follow-up was identified as the only predictor for a worse outcome in these patients.

Summary: The clinical and prognostic implications of a hypertensive response to exercise after repair of coarctation of the aorta remain controversial. In this study, nearly half of the adults with repaired coarctation of the aorta had a hypertensive response to exercise. Male sex, absence of a bicuspid aortic valve, and elevated resting systolic blood pressure were associated with increased peak exercise blood pressure. Peak exercise blood pressure independently predicted hypertension during follow-up.

Summary: This study found that compared to receiving primary care only, ACHD patients receiving cardiology care had lower mortality and major complication rates. However, it is concerning that even in a high resource setting, almost half of ACHD patients are not receiving regular cardiac care. More efforts are needed to ensure appropriate ACHD care for patients.

Summary: CAAs are a group of congenital conditions characterized by abnormal origin or course of the 3 main epicardial coronary arteries. While they have been identified as a common underlying condition in young athletes, their recognition among adults has increased with the widespread use of invasive and noninvasive coronary imaging. The clinical correlates and therapeutic choices for CAAs remain poorly understood, with studies suggesting that multimodality imaging may play a key role in management.

Summary: This study aimed to elucidate the molecular basis of laterality disorders and associated congenital heart defects in a cohort of 30 unrelated probands of Arab-Muslim descent using next-generation sequencing techniques. A molecular diagnosis was reached for 56.7% of the probands, with known genes associated with heterotaxy and/or primary ciliary dyskinesia identified, as well as a novel recessive monogenic cause for heterotaxy in humans. Additionally, variants of uncertain significance were found in three probands.

Summary: This study investigated the association between maternal exposure to fine particulate matter (PM2.5) and fetal congenital heart defects (CHDs) in China. The results showed that higher maternal PM2.5 exposure, especially during the preconception period, increased the risk of certain types of CHDs in offspring. The findings highlight the importance of improving air quality in China and other highly polluted regions for CHD prevention.

Summary: This study analyzed the interactions between underlying and multiple causes of death related to heart failure in Brazil from 2006 to 2016. It found that the frequency of heart failure as a multiple cause of death was nearly three times higher than as an underlying cause of death. The study highlights the importance of considering heart failure as a complex syndrome with multiple components in analyzing mortality trends for public health management programs.

Summary: This study analyzed the characteristics and outcomes of waitlisted patients for heart transplant in the US between 2018 and 2020, finding that patients using exception status were more likely to have nonischemic heart failure and blood type O. There were significant variations in exception status utilization across different regions and centers, and listing by exception criteria was associated with a higher incidence of heart transplantation compared to listing by standard criteria.

Summary: This study evaluated the impact of left-to-right shunt in type II atrial septal defect patients on left and right ventricular volumes, function, and strain using cardiovascular magnetic resonance imaging. The findings showed that the shunt resulted in RV overfilling, LV underfilling, and impaired systolic RV performance associated with longitudinal dysfunction of the interventricular septum.

Summary: There is a lack of research on the association between structured diabetes self-management education (DSME) and glycemic control in low- and middle-income countries (LMICs). However, existing evidence suggests that structured DSME is associated with reductions in glycated hemoglobin in these countries, particularly in sub-Saharan Africa.

Summary: This study compiled evidence of changes in blood glucose during and after tuberculosis treatment in previously normoglycaemic patients. It was found that elevated blood glucose in normoglycaemic patients decreased by the end of treatment. Patients with hyperglycaemia at baseline were more likely to develop cavitary lung lesions and experienced poor treatment outcomes and higher post-treatment mortality.

Summary: This article aims to generate consensus-based recommendations for the prevention and screening of cardiometabolic and kidney diseases following hypertensive disorders in pregnancy (HDP) in low- and middle-income countries (LMICs). The recommendations provide practical and scalable approaches for effective screening and prevention of cardiometabolic disease following HDP in LMICs. Monitoring and evaluation of implementation of these recommendations provide opportunities for reducing the escalating burden of noncommunicable diseases in LMICs.

Summary: RVGCW is a novel parameter that provides an integrative analysis of RV systolic function and correlates more closely with invasively measured stroke volume and stroke volume index than other standard echocardiographic parameters. In patients with HFrEF, RVGWI, RVGCW, and RVGWE values were significantly lower, while values of RVGWVV were significantly higher compared to a healthy cohort.
