High mortality among heart failure patients treated with antidepressants

Background: This study was designed to assess whether pharmacologically treated depression was associated with increased mortality risk in systolic heart failure (SHF) patients. Methods: Patients (n=3346) with SHF (left ventricular ejection fraction <= 0.45) and primarily New York Heart Association (NYHA) classes II-III (78%) were recruited from a clinical database used in 20 heart failure clinics in Denmark. The association between pharmacologically treated depression identified by at least one prescription of an antidepressant and mortality risk was evaluated. Results: Follow-up time was 540 days (range: 30-1600 days). 539 patients died. For 243 patients (7%) an antidepressant had been prescribed at least once. In a Cox Proportional Hazard Model, pharmacologically treated depression was associated with a 49% increased mortality risk (Hazard ratio: 1.49, 95% confidence interval: 1.03-2.16) after adjustment for traditional confounders. Three months after the baseline visit in the heart failure clinic, these patients received lower doses of beta-blockers than patients without antidepressant therapy (p=0.006). Female sex (p<0.001) and NYHA classes III-IV (p=0.007) were associated with the prescription of an antidepressant. Conclusions: Our analyses suggest that pharmacologically treated depression is associated with a 49% increased mortality risk, and that these high-risk patients receive lower doses of beta-blockers than patients with no antidepressant therapy. Summary: Acute heart failure (AHF) represents a wide range of disease states caused by the interaction between acute precipitants and a patient's underlying cardiac substrate and comorbidities. Valvular heart disease (VHD) is often associated with AHF. The severity of VHD and its correlation with symptoms can be difficult to assess in AHF patients due to various factors. Evidence-based interventions targeting VHD in AHF settings are yet to be identified, making current guidelines elusive. This scientific statement aims to provide an overview of the epidemiology, pathophysiology, and treatment approach for AHF patients with VHD.

Summary: This article reviews the clinical consensus on the use of inotropic support in patients with advanced heart failure. Current guidelines recommend inotropic use only in cases of acute decompensated heart failure with organ malperfusion or shock. However, inotropic support may be justified in other cases of advanced heart failure without severe acute decompensation. The article examines the clinical evidence supporting the use of inotropes in these situations, including patients with persistent congestion, systemic hypoperfusion, or advanced heart failure requiring palliation, and those undergoing left ventricular assist device implantation or heart transplantation.

Prevalence and clinical features of heart failure in Greenland

Summary: Heart Failure (HF) is a significant burden for healthcare worldwide. This study aims to estimate the prevalence of HF and describe the characteristics of patients with HF in Greenland. The overall prevalence was 1.1%, higher among men than women, and highest among men above 84 years. More than half of the patients were obese and/or smokers, and a low prevalence of ischaemic heart disease (IHD) was observed.

This standardized assay may provide important information on immune function post-SOT.

Adherence to Pharmacotherapies After Heart Transplantation in Relation to Multimorbidity and Socioeconomic Position: A Nationwide Register-Based Study

Summary: This study examined the impact of multimorbidity and socioeconomic position on treatment patterns and adherence to pharmacological therapies in heart transplantation recipients. The study found that recipients with multiple chronic diseases had higher prevalence of treatment with antihypertensive pharmacotherapies and loop diuretics. Additionally, socioeconomically disadvantaged recipients had lower numbers of patients treated with and adherence to certain cost-free medications.
