The receptor activator of NF-κB ligand-osteoprotegerin system in pulpal and periapical disease

Aim To summarize the collective in vitro, in vivo and clinical evidence of the involvement of the receptor activator of NF-kappa B ligand (RANKL)-osteoprotegerin (OPG) system, a system of two molecules controlling osteoclast differentiation and hard-tissue resorption, in pulpal and periapical pathophysiology. Methodology A systematic search related to RANKL and/or OPG and pulp or periapical disease was conducted on Medline, Biosis, Cochrane, Embase and Web of Science databases using keywords and controlled vocabulary. No language restriction was applied. Two independent reviewers first screened titles and abstracts and then the full texts that were initially included. The reference lists of the identified publications were examined for additional titles. Results A total of 33 papers were identified. In vitro studies (N - 11) revealed that pulpal cells can be stimulated by various inflammatory agents to produce RANKL, whilst many studies did not consider the RANKL/OPG ratio. Animal studies (N = 9) mostly focused on the time course and development of periapical lesions in relation to the RANKL-OPG system. Levels of RANKL and OPG in the necrotizing pulp were not investigated. Human studies (N - 13) showed a steady-state expression of OPG in the odontoblast layer. Conflicting results have been reported regarding the role of RANKL in active apical periodontitis, again because the correlation of this molecule with its inhibitor (OPG) was often disregarded. Conclusions There is relatively little information currently available that would highlight the specific role of RANKL and OPG in pulpal and periapical disease. OPG may play a protective role against internal resorption, whilst an increased periapical RANKL/OPG ratio might indicate bone resorption.

MMP-9 Levels and NaOCl Lavage in Randomized Trial on Direct Pulp Capping

Extracellular Vesicle Subproteome Differences among Filifactor alocis Clinical Isolates

Assessment of Irrigant Agitation Devices in Simulated Closed and Open Root Canal Systems

Summary: This study compared the in vitro efficacy and safety of different irrigant activation devices in apically closed and open root canal systems. The results showed that ultrasonic agitation was more effective in cleaning the central area of the root canal, while it had a lower effect in cleaning the simulated periapical lesion. Therefore, ultrasonic agitation was found to be more targeted and safer after the administration of sodium hypochlorite.

Periodontal microbiology and microbial etiology of periodontal diseases: Historical concepts and contemporary perspectives

Nanostructured Ag-Bioglass Implant Coatings with Antibacterial and Osteogenic Activity

Summary: Bone implants often fail due to bacterial colonization and loose biofilm infections. A novel nanoparticle coating composed of silver and bioglass has been developed to address these issues. The coating is produced using a scalable and reproducible process and exhibits both antibacterial and osteogenic properties. The silver-ion release inhibits biofilm formation, while the coating maintains high biocompatibility.

Age-Related Subgingival Colonization of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis and Parvimonas micra-A Pragmatic Microbiological Retrospective Report

Summary: The aim of this study was to compare the prevalence and proportions of three bacterial species in periodontitis pocket samples from young and old patients. The results showed that the prevalence of Aggregatibacter actinomycetemcomitans was higher in young patients, while Porphyromonas gingivalis was similarly distributed in both age groups. Parvimonas micra was more prevalent in old patients. Furthermore, the proportion of samples containing A. actinomycetemcomitans and P. micra together was higher than when P. gingivalis was present.

Precision-milled simulated curved root canals in bovine dentine for the assessment of chemo-mechanical root canal preparation

Summary: This study aimed to develop a standardized curved root canal model in bovine dentine and compare its behavior to resin in standard plastic training blocks during chemo-mechanical preparation. The study found that dentine had higher microhardness compared to resin, and dentine exhibited greater canal transportation, which was slightly increased by EDTA. The results are important for testing and comparing materials and concepts of chemo-mechanical root canal instrumentation.

Aggregatibacter actinomycetemcomitans and Filifactor alocis: Two exotoxin-producing oral pathogens

Summary: Periodontitis is an imbalanced disease caused by the interaction between the microbial ecosystem and the host immune response. The disease-associated microbial community consists of known oral pathogens and newly emerging species, which grow within the periodontal pocket and evade the host immune response. This review focuses on the virulence factors and potential pathogenicity of these oral pathogens in periodontitis, highlighting the recent discovery of virulence factors in Filifactor alocis and comparing them with those associated with Aggregatibacter actinomycetemcomitans. Synergistic and/or mutualistic interactions between these oral pathogens might contribute to disease progression.
