Food allergy and food intolerance: diagnosis and treatment

Food allergy is a matter of concern because it affects about 0.5-3.8% of the paediatric population and 0.1-1% of adults, and as well may cause life-threatening reactions. Skin prick testing with food extracts and with fresh foods, the measurement of food-specific IgE, elimination diets and a double-blind, placebo-controlled food challenge are the main diagnostic procedures; many non-validated procedures are available, creating confusion among patients and physicians. The treatment of food allergy is still a matter of debate. Antihistamines, corticosteroids and, if necessary (in case of anaphylaxis), epinephrine, are the drugs of choice for the treatment of symptoms of food allergy. Sodium cromolyn may be used prophylactically even though there are no controlled studies certifying its efficacy. The only etiologic treatment of food allergy is specific desensitization. Sublingual-oral-specific desensitization has been used by our group for the treatment of food-allergic patients with a high percentage of success.

Summary: In Italian cypress pollen-allergic patients, monosensitization to peamaclein appears to be rare, with most patients reacting to peach, although other Rosaceae fruits and citrus fruits may also be offending foods. Aside from peamaclein, peach and cypress pollen likely share other cross-reacting allergens.

Contrast Medium Hypersensitivity: A Large Italian Study with Long-Term Follow-Up

Summary: This study compared hypersensitivity reactions to different types of contrast media and explored the possibility of new reactions in a long-term follow-up. It was found that patients with hypersensitivity reactions to gadolinium-based contrast media had a higher prevalence of asthma, food allergy, non-allergic drug hypersensitivity, and neurologic disease. Some patients reported new reactions despite negative skin test results.

Telemedicine technologies have become an important tool in managing adolescent asthmatics during the COVID-19 pandemic.

Epinephrine Auto-Injector Prescription and Use: A Retrospective Analysis and Clinical Risk Assessment of Adult Patients Sensitized to Lipid Transfer Protein

Summary: Lipid transfer proteins (LTPs) are common allergens found in plant food and are the main cause of food allergies in adults in the Mediterranean basin. This study investigated the use of prescribed epinephrine auto-injectors in LTP patients and the appropriateness of their prescription. It also examined the occurrence of new food reactions, the need for emergency services, and possible predictive factors for further food reactions in these patients. The study found that most LTP patients experienced local symptoms in further reactions, with a small percentage requiring emergency care and only one patient using the epinephrine auto-injector. Patients with a history of anaphylaxis were more likely to seek emergency services during the follow-up period, and there was an association between platanus pollen sensitization and the severity of further reactions. The study also highlighted the need for follow-up to monitor patients over time and assess the actual use of prescribed epinephrine auto-injectors.

Mepolizumab Improves Outcomes of Chronic Rhinosinusitis with Nasal Polyps in Severe Asthmatic Patients: A Multicentric Real-Life Study

Summary: This study evaluated the effectiveness of mepolizumab in treating comorbid CRSwNP and severe asthma. The results showed significant improvements in SNOT-22 score, NP score, and blood eosinophil count after 12 months of treatment. Some patients experienced improvements in both clinical and endoscopic outcomes. Responders had higher baseline SNOT-22 score compared to nonresponders. Further studies are needed to supplement these preliminary findings.

Summary: Celiac disease is an intestinal disease triggered by gluten ingestion in genetically predisposed individuals. It is mediated by Th17, IL-21, and IL-17A lymphocytes, resulting in intestinal inflammation and altered mucosal architecture. B cells produce antibodies against tissue transglutaminase and deamidated gliadin. Besides the adaptive immune response, the role of innate immunity cells such as alarmins (DAMPs) is poorly understood. Our study aims to evaluate the evidence regarding the involvement of ILCs and alarmins in celiac disease and their potential diagnostic and therapeutic implications.
