Summary: There has been significant progress in understanding the gut microbiome in health and disease over the past decade. The gut microbiome interacts with the immune system and plays a crucial role in defending the host against pathogens. However, disturbances in the gut microbiome due to disease or interventions like antibiotic treatment can have negative consequences. This review focuses on the potential role of probiotics in the ICU, summarizes evidence from randomized clinical trials, and discusses challenges and future prospects.

Summary: The study revealed that 2019-nCoV pneumonia should be considered a systemic disease. Lymphocyte percentage, blood urea nitrogen, calcium, myohemoglobin, and IL-6 levels are closely associated with patient prognosis. The survival rate of patients in the severe group was higher than that of patients in the critical group.

Summary: In recent decades, invasive candidiasis (IC) has become a significant cause of morbidity, mortality, and economic burden in the intensive care unit (ICU). The epidemiology of IC remains a difficult and unsolved puzzle for the literature. IC burden estimation is challenging due to various definitions and limitations in case-finding methodologies. Candidemia and intra-abdominal candidiasis (IAC) are the predominant types of IC in the ICU, with an increasing incidence of candidemia, particularly in elderly or immunosuppressed patients, and a shift towards non-albicans species worldwide.

Summary: In December 2020, Candida auris emerged in Brazil, with the first two patients colonized in the same COVID-19 intensive care unit. Antifungal susceptibility testing showed low minimal inhibitory concentrations for various drugs. Microsatellite typing revealed clonal strains belonging to the South Asian clade of C. auris. The hypothesis is that the species was introduced several months before the first recognized case and/or emerged locally in the coastline Salvador area during the COVID-19 pandemic.

Summary: This study used mNGS and conventional tests to detect 80 pathogens in 262 children with suspected pulmonary infections. RSV, Staphylococcus aureus, and rhinovirus were the most frequently detected pathogens. Co-infection was common, with bacterial-viral agents most commonly detected together. The study highlights the importance of using advanced diagnostic techniques like mNGS to improve our understanding of the microbial epidemiology of severe pneumonia in pediatric patients.

Summary: For mechanically ventilated patients with pneumonia, critical care in the ICU was associated with a 7.2% decrease in 30-day in-hospital mortality compared to care in the HDU. The study utilized propensity score matching analysis, multivariable regression, overlap weighting, and instrumental variable analyses to evaluate the robustness of the findings.

Summary: This study revealed that persistent cardiovascular failure significantly impacts the outcomes of patients with ICU-acquired pneumonia (ICUAP), resulting in higher mortality rates. Persistent CVF may lead to treatment failures and bacterial superinfections, highlighting the importance of timely recognition to optimize patient management.

Summary: The study in India focused on MITS of the brain after neonatal death, showing good diagnostic yield in identifying neurological insults. MITS proved to be an accurate and adequate alternative to complete diagnostic autopsy, especially in low-resource settings where obtaining consent for more invasive procedures is challenging.

Summary: This study aimed to evaluate the performance of ICU severity scores in predicting mortality in pneumonia patients. The findings showed that commonly used severity scores such as SAPS 3, CURB-65, CRB-65, and qSOFA had poor predictive ability in this context. A novel pneumonia-specific ICU severity score (Shock score) was developed and found to outperform existing scores in identifying patients at highest risk of ICU death.

Summary: This study investigates the risk factors and prognostic indicators for severe aspiration pneumonia (SAP) in ICU patients. The results suggest that a low platelet count is an independent risk factor affecting the prognosis of death. Therefore, platelet level at admission may serve as a readily available marker for assessing the prognosis of patients with SAP.

Summary: In this study, the skin microbiota of ICU patients with severe pneumonia was analyzed using 16S ribosomal ribonucleic acid sequencing. The results showed a significant decrease in the diversity of skin bacteria in the ICU patient group compared to the healthy control group, and differential abundance of certain bacterial species. These findings suggest a strong association between the skin microbiota and healthcare-associated infections in ICU patients with severe pneumonia, highlighting the importance of prevention and control of skin bacterial pathogens.

Summary: This study found that ICU-acquired pneumonia caused by highly antimicrobial-resistant (HAMR) bacteria is associated with a significantly increased ICU mortality rate. The research also showed that patients with pneumonia caused by HAMR bacteria had longer ICU length of stay and mechanical ventilation duration.
