Summary: In adult patients under VA-ECMO, a simplified 4-frontal electrode EEG montage interpreted by an intensivist, detected common EEG patterns associated with poor outcomes, with a performance similar to that of a standard EEG montage interpreted by expert neurophysiologists.

Summary: This study analyzed adult patients admitted to intensive care units in Greater Paris for ECMO treatment during the early stages of the COVID-19 pandemic. Most patients were male and prone positioned before ECMO. The 90-day survival rate after ECMO treatment was 46%, with the main causes of death being multiorgan failure and septic shock.

Effect of Moderate Hypothermia vs Normothermia on 30-Day Mortality in Patients With Cardiogenic Shock Receiving Venoarterial Extracorporeal Membrane Oxygenation A Randomized Clinical Trial

Slutsky, Daniel Brodie, Jean-Luc Fellahi, Alexandre Ouattara, Alain Combes

Summary: A randomized clinical trial was conducted to investigate whether early use of moderate hypothermia during venoarterial ECMO treatment improves mortality in patients with cardiogenic shock. The results showed that early application of moderate hypothermia for 24 hours did not significantly increase survival compared with normothermia.

Matthay, Sophie Provenchere, Erwan Flecher, Andre Vincentelli

Summary: This study reported the characteristics, management, and outcomes of COVID-19 patients receiving extracorporeal membrane oxygenation in France. The in-hospital mortality rate was higher than previous reports, but nearly half of the patients survived. Factors associated with in-hospital mortality included the use of neuromuscular blockade, duration of ventilation, age, and total bilirubin levels at cannulation.

Combined Bacterial Meningitis and Infective Endocarditis: When Should We Search for the Other When Either One is Diagnosed?

Guillaume Beraud, Sarah Tubiana, Marie-Line Erpelding, Vincent Le Moing, Catherine Chirouze, Isabelle Gorenne, Pauline Manchon, Pierre Tattevin, Veronique Vernet, Emmanuelle Varon, Bruno Hoen, Xavier Duval

Summary: The study described patients with coexisting infective endocarditis and bacterial meningitis, finding that the main presentation was meningitis, with most common pathogens being Streptococcus pneumoniae and Staphylococcus aureus. The results of this study are important for the identification and treatment of patients with dual infections.

Bleeding and thrombotic events in patients with severe COVID-19 supported with extracorporeal membrane oxygenation: a nationwide cohort study

Alexandre Mansour, Erwan Flecher, Matthieu Schmidt, Bertrand Rozec, Isabelle Gouin-Thibault, Maxime Esvan, Claire Fougerou, Bruno Levy, Alizee Porto, James T. Ross, Marylou Para, Sabrina Manganiello, Guillaume Lebreton, Andre Vincentelli, Nicolas Nesseler

Summary: This study described the incidence and risk factors of bleeding and thrombotic events in COVID-19 patients receiving ECMO, and evaluated their impact on in-hospital mortality. The results showed that bleeding events were associated with mortality, especially intracranial hemorrhage with the highest risk of death, while thrombotic events were less frequent and not associated with mortality. The length of ECMO support was a risk factor for both bleeding and thrombosis.

Elucidating tricuspid Doppler signal interpolation and its implication for assessing pulmonary hypertension

Seraina A. Dual, Constance Verdonk, Myriam Amsallem, Jonathan Pham, Courtney Obasohan, Patrick Nataf, Doff B. McElhinney, Alisa Arunamata, Tatiana Kuznetsova, Roham Zamanian, Jeffrey A. Feinstein, Alison Marsden, Francois Haddad

Summary: Doppler echocardiography is central in evaluating pulmonary hypertension. A study using cubic polynomial interpolation of TR waveforms found strong correlation between third derivative interpolation and RVSP estimates in patients with PAH and advanced lung disease, suggesting potential for improved SPAP assessment.
