Background: The use of complementary and alternative medicine in inflammatory bowel disease (IBD) has been extensively studied. However, the use of probiotics and prebiotics is poorly documented, despite evidence of efficacy of particular probiotic strains in specific forms of IBD. Methods: A case-control study comprising interviewer-administered questionnaires was conducted in IBD patients and healthy controls. Data regarding use and knowledge of probiotics and prebiotics, demographic, and clinical information were collected. Results: In total, 334 participants (234 IBD, 100 controls) were interviewed. Significantly more IBD patients than controls had ever used probiotics to manage their health (Crohn's disease [CD] 43%, ulcerative colitis [UC] 51%, controls 21%, P < 0.001). Prebiotic use was negligible. On logistic regression analysis, having UC (odds ratio [OR] 4.30, 95% confidence interval [CI] 2.27-8.12) or CD (OR 3.05, 95% Cl 1.66-5.60) were the strongest predictors of probiotic use. Within IBD patients the strongest predictor of probiolic use was current steroid use (OR 2.4, 95% Cl 1.11-5.18). IBD patients had greater probiotic knowledge scores than controls (P = 0.003), although 20% of IBD probiotic users could not provide a definition of a probiotic. Less than half of IBD probiotic users discussed probiotic use with healthcare professionals, with commercial advertising being the primary source of information. Conclusions.: Patients with IBD use probiotics to manage their health but frequently choose strains without evidence of efficacy in IBD. Patients rely on nonclinical sources of information and often do not disclose probiotic use to healthcare professionals. Conventional healthcare providers should inquire about probiotic use among, their patients and offer evidence-based advice.

Nopal fiber (Opuntia ficus-indica) improves symptoms in irritable bowel syndrome in the short term: a randomized controlled trial

Jose M. Remes-Troche, Hector Taboada-Liceaga, Samantha Gill, Mercedes Amieva-Balmori, Megan Rossi, Gildardo Hernandez-Ramirez, Jose F. Garcia-Mazcorro, Kevin Whelan

Summary: Nopal fiber supplementation at doses of 20 and 30 g/day was associated with short-term improvement in IBS symptoms, warranting a fully powered clinical trial of longer duration with symptomatic, physiological, and microbiological endpoints.

Relationship Between Skeletal Muscle Area and Density and Clinical Outcome in Adults Receiving Venovenous Extracorporeal Membrane Oxygenation

Danielle E. Bear, Liisa MacGowan, Maria Elstad, Zudin Puthucheary, Bronwen Connolly, Rebeka Wright, Nicholas Hart, Stephen Harridge, Kevin Whelan, Nicholas A. Barrett, Luigi Camporota

Summary: This study investigated the prevalence of low skeletal muscle index and density in adults with severe respiratory failure and their associations with clinical outcomes. The findings suggest that low skeletal muscle index was associated with a longer duration of venovenous extracorporeal membrane oxygenation, while higher skeletal muscle density was associated with improved ICU survival.

Food Additive Emulsifiers and Their Impact on Gut Microbiome, Permeability, and Inflammation: Mechanistic Insights in Inflammatory Bowel Disease

Aaron S. Bancil, Alicia M. Sandall, Megan Rossi, Benoit Chassaing, James O. Lindsay, Kevin Whelan

Summary: The global burden of inflammatory bowel disease [IBD] has increased over the 21st century, with factors such as a 'Western diet' and dietary emulsifiers potentially contributing to the rise in prevalence. These factors may trigger the pathogenesis of IBD through various mechanisms.

Food-related quality of life is impaired in inflammatory bowel disease and associated with reduced intake of key nutrients

Kevin Whelan, Trevor Murrells, Myfanwy Morgan, Fraser Cummings, Catherine Stansfield, Anne Todd, Shaji Sebastian, Alan Lobo, Miranda C. E. Lomer, James O. Lindsay, Wladyslawa Czuber-Dochan

Summary: In patients with inflammatory bowel disease (IBD), impaired food-related quality of life (FR-QoL) is prevalent and associated with factors such as recurrent disease flares, reduced IBD-specific QoL, and greater IBD-related distress. Patients with poorer FR-QoL tend to have lower intakes of key nutrients important to IBD, including fiber, calcium, phosphorus, and magnesium, which are essential for gut health and bone mineralization.

Fermented foods: Availability, cost, ingredients, nutritional content and on-pack claims

Kevin Whelan, Nicola Jones

Summary: This study investigated the availability, cost, ingredients, nutritional content, and on-pack claims of commercially-produced fermented foods in the UK. The research found that there were differences in the number of products, cost, and nutritional content among different categories of fermented foods. Commercially-produced fermented foods are diverse in their cost, nutritional content, ingredients, and use of on-pack claims.

Prebiotic fructans have greater impact on luminal microbiology and CD3+T cells in healthy siblings than patients with Crohn's disease: A pilot study investigating the potential for primary prevention of inflammatory bowel disease

Charlotte R. Hedin, Neil E. McCarthy, Petra Louis, Freda M. Farquharson, Sara McCartney, Andrew J. Stagg, James O. Lindsay, Kevin Whelan

Summary: This study found that supplementation with prebiotic oligofructose/inulin did not significantly impact calprotectin levels in patients with CD and their siblings, but had a more marked prebiotic effect in healthy siblings. Further research should focus on dietary intervention, including with prebiotics, in the primary prevention of CD.

Particle Size Distribution and Predicted Lipid Bioaccessibility of Almonds and the Effect of Almond Processing: A Randomised Mastication Study in Healthy Adults

Alice C. C. Creedon, Estella S. S. Hung, Eirini Dimidi, Terri Grassby, Sarah E. E. Berry, Kevin Whelan

Summary: Almonds contain unsaturated lipids, which are beneficial for human health. The unique properties of almonds affect their particle size distribution after chewing, making it difficult for the body to fully digest the almond lipids. Commercial grinding of almonds into flour changes the particle size distribution and slightly increases the predicted bioaccessibility of lipids.
