Since 2006, several projects have been initiated in Europe to describe the distribution and characteristics of infection control programs in long-term care facilities and to quantify the burden of infections, antimicrobial resistance, and antimicrobial use in long-term care facilities. This article summarizes the results and activities that are underway.

Monitoring the community use of antibiotics in Italy within the National Action Plan on antimicrobial resistance

Summary: In Italy, both antibiotic consumption and bacterial resistance rates are higher than in other European countries. The study found that in 2018, the consumption of antibiotics in Italy was 16.1 DDD per 1000 inhabitants per day, with higher rates in the southern region. Consumption was higher in extreme age groups and winter season, with peaks in flu syndromes.

Summary: This study evaluated the incidence, attributable deaths, and burden of the most significant HAIs in Italy in 2016. The results showed a substantial burden of HAIs in Italy, with bloodstream infections accounting for the majority of total DALYs. The findings emphasize the importance of reducing the burden of HAIs through infection prevention and control efforts.

Summary: This study focuses on describing the trends of mcr-positive Enterobacterales in humans in the Emilia-Romagna region of Italy based on laboratory surveillance. The study found a significant decrease in the number of mcr-positive isolates from the first half of 2018 to the whole year of 2020, primarily due to horizontal transfer of mcr genes through plasmids or dissemination of transposable elements. Overall, there has been a substantial reduction in the circulation of mcr-1 plasmid genes in the Emilia-Romagna region.

How Many Cancer Patients Need Palliative Care? A Population-Based Study

General and COVID-19-Related Mortality by Pre-Existing Chronic Conditions and Care Setting during 2020 in Emilia-Romagna Region, Italy

Summary: In 2020, Italy experienced an increase in mortality due to the COVID-19 pandemic, with significant variations among regions and demographic groups. This study in the Emilia-Romagna region found that excess deaths were higher during the first wave of the pandemic, particularly among men and in March. While metabolic and neuropsychiatric diseases were more prevalent among deceased individuals in 2020, excluding COVID-19 related deaths led to a decrease in differences in mortality risk estimates. Understanding the impact of the pandemic on mortality, especially considering underlying frailties, is crucial in this evolving situation.

Increasing Number of Cases Due to Candida auris in North Italy, July 2019-December 2022

Summary: Candida auris was first detected in Italy in July 2019, with one additional case reported in January 2020. Nine months later, a large number of cases were reported in northern Italy. A total of 361 cases were detected between July 2019 and December 2022 in various regions of Italy, with 146 deaths. The majority of cases were considered to be colonization, and only one had a history of travel abroad. Most isolates showed resistance to fluconazole.

Effectiveness of community hospital post-acute care on mortality, re-admission, institutionalization, and activation of a home care programme in Emilia-Romagna region, Italy

Summary: This study aimed to evaluate the impact of transferring patients to Community Hospitals (CHs) on mortality, re-admission, institutionalization, and activation of a home care program in the Emilia-Romagna Region (Italy) after acute hospitalization. The findings showed that patients transferred to CHs had a lower risk of dying but an increased risk of readmission. The risk of institutionalization was lower in the CH exposed group, except for patients with certain chronic diseases. The activation of a home care program was slightly higher for those transferred to a CH.

Reducing antibiotic prescriptions in children is not associated with higher rate of complications

Summary: This study observed a reduction in antibiotic prescriptions for acute otitis media and streptococcal pharyngitis in children in the Emilia-Romagna region of Italy after implementing the ProBA project guidelines. The rates of complications such as acute mastoiditis and acute rheumatic fever did not increase as a result. Administrative databases were found to be useful for monitoring projects and ensuring healthcare professionals adherence to guidelines.
