Two Time-Series Analyses of the Impact of Antibiotic Consumption and Alcohol-Based Hand Disinfection on the Incidences of Nosocomial Methicillin-Resistant Staphylococcus aureus Infection and Clostridium difficile Infection

OBJECTIVE. To determine the impact of antibiotic consumption and alcohol-based hand disinfection on the incidences of nosocomial methicillin-resistant Staphylococcus aureus (MRSA) infection and Clostridium difficile infection (CDI). METHODS. Two multivariate time-series analyses were performed that used as dependent variables the monthly incidences of nosocomial MRSA infection and CDI at the Freiburg University Medical Center during the period January 2003 through October 2007. The volume of alcohol-based hand rub solution used per month was quantified in liters per 1,000 patient-days. Antibiotic consumption was calculated in terms of the number of defined daily doses per 1,000 patient-days per month. RESULTS. The use of alcohol-based hand rub was found to have a significant impact on the incidence of nosocomial MRSA infection (P < .001). The multivariate analysis (R(2) = 0.66) showed that a higher volume of use of alcohol-based hand rub was associated with a lower incidence of nosocomial MRSA infection. Conversely, a higher level of consumption of selected antimicrobial agents was associated with a higher incidence of nosocomial MRSA infection. This analysis showed this relationship was the same for the use of second-generation cephalosporins (P = .023), third-generation cephalosporins (P = .05), fluoroquinolones (P = .01), and lincosamides (P = .05). The multivariate analysis (R(2) = 0.55) showed that a higher level of consumption of third-generation cephalosporins (P = .008), fluoroquinolones (P = .084), and/or macrolides (P = .007) was associated with a higher incidence of CDI. A correlation with use of alcohol-based hand rub was not detected. CONCLUSION. In 2 multivariate time-series analyses, we were able to show the impact of hand hygiene and antibiotic use on the incidence of nosocomial MRSA infection, but we found no association between hand hygiene and incidence of CDI.

Cost-benefit analysis of the CoCare intervention to improve medical care in long-term care nursing homes: an analysis based on claims data

Summary: This study aimed to optimize nursing staff-physician collaboration in nursing homes to reduce hospital admissions and ambulance transportations, thereby reducing healthcare costs. The results showed that the intervention group had reduced medical service costs, hospital stays, and patient transports, leading to cost-savings in inpatient services. Additionally, more services were billed by general practitioners in the intervention group, resulting in additional costs.

Impact of the COVID-19 pandemic on aortic valve replacement procedures in Germany

Summary: Due to the COVID-19 pandemic, millions of elective procedures in Germany, including surgical aortic valve replacement (sAVR) and transcatheter aortic valve replacement (TAVR), were postponed. The overall reduction in AVR procedures in 2020 was 19.07%, with sAVR being more affected than TAVR, and the impact was less pronounced during the second wave of the pandemic.

Summary: This study compared the outcomes of planned and unplanned use of rotational atherectomy (RA) in patients with severely calcified coronary lesions. The results showed that unplanned RA had lower rates of complications and better clinical outcomes compared to planned RA during the procedure and at 1-year follow-up. Therefore, RA can be considered as a alternative treatment strategy for lesions that cannot be treated with conventional methods.

Tick-borne encephalitis virus IgG antibody surveillance: vaccination- and infection-induced seroprevalences, south-western Germany, 2021

Summary: This study investigated the epidemiology of tick-borne encephalitis virus (TBEV) in a highly endemic district in southwestern Germany. The results showed that TBEV infections often have mild or asymptomatic courses, leading to inaccurate epidemiological data. Using the NS1 IgG ELISA, a high prevalence of TBEV infections and virus-induced antibodies were detected in the population, indicating a potential high incidence rate.

Serological Protection Rates against TBEV Infection in Blood Donors from a Highly Endemic Region in Southern Germany

Summary: The study in Ortenaukreis, Germany showed that the average serological protection rate of female blood donors was 57% and that of male blood donors was 52%. Compared to official data, the serological protection rate exceeded the average vaccination rate by 23.2% for females and 21% for males. This indicates a potentially longer persistence of TBE vaccination-induced antibody titers than previously assumed.

Treatment of pure aortic regurgitation using surgical or transcatheter aortic valve replacement between 2018 and 2020 in Germany

Summary: Through the analysis of health records in Germany from 2018 to 2020, it was found that transcatheter aortic valve replacement (TAVR) is a viable alternative surgical method for pure aortic regurgitation, especially self-expanding transfemoral TAVR. TAVR treatment has overall low in-hospital mortality and complication rates.

Impact of CoCare, a Complex Model Intervention, on medical care in long-term care nursing homes in Germany: An overview of the outcome and process evaluation

Summary: This article presents the results of an intervention called CoCare, which aimed to improve coordination of medical care and reduce hospitalization rates and ambulance transportation in nursing homes. The intervention was found to be effective in lowering costs and reducing hospital admissions and ambulance transportation. Both nursing staff and physicians perceived positive changes in care and assessed the benefits of the intervention. However, these changes were not perceived by the residents.

Catheter based left atrial appendage closure in-hospital outcomes in Germany from 2016 to 2020

Summary: In Germany, endocardial occlusion is the major catheter-based left atrial appendage closure procedure, but there has been no improvement in in-hospital safety over the past five years. The occurrence of major adverse cardiac and cerebrovascular events and pericardial puncture during hospitalization is independent of the number of procedures performed in each center. The epicardial loop stitch procedure is utilized in a limited number of centers, and it has different risks compared to endocardial occlusion.

Evolving trends in the surgical therapy of patients with endometrial cancer in Germany: analysis of a nationwide registry with special emphasis on perioperative outcomes

Summary: This study analyzed the evolving trends of surgical therapy for endometrial cancer (EC) in Germany. The results showed that minimal-invasive surgery has become the standard procedure for EC patients, with better in-hospital outcomes compared to laparotomy. The use of robotic-assisted laparoscopic surgery is also increasing, with a comparable in-hospital safety profile to conventional laparoscopy.

Routine versus selective near-infrared spectroscopy-guided shunting during carotid eversion endarterectomy

Summary: The aim of this study was to compare outcomes of routine shunting to near-infrared spectroscopy (NIRS)-guided shunting in patients undergoing eversion endarterectomy (EEA) under general anesthesia. The study found that, in patients undergoing EEA under general anesthesia, NIRS-guided selective shunting was associated with a lower incidence of perioperative stroke compared to routine shunting.
