Daptomycin underexposure in a young intravenous drug user who was affected by life-threatening Staphylococcus aureus-complicated skin and soft tissue infection associated with bacteraemia

We describe the case of an intravenous drug user affected by life-threatening Staphylococcus aureus-complicated skin and soft tissue infection with associated bacteraemia who, while on replacement therapy with methadone, required 11 mg/kg/day daptomycin to achieve trough (C-min) and peak (C-max) plasma levels similar to those observed with the standard dosage of 6 mg/kg in healthy volunteers (mean +/- A standard deviation: C-min 12.35 +/- A 0.80 mg/L, C-max 63.90 +/- A 8.71 mg/L). Clinical pharmacological advice based on real time therapeutic drug monitoring may be helpful for optimizing daptomycin exposure in these patients. Physicians should take into account that dosages much higher than the standard ones may be needed, probably as a consequence of augmented drug clearance.

Clinical efficacy of renal dosing adjustments of ceftazidime-avibactam in patients affected by carbapenem-resistant Gram-negative infections: A systematic review and meta-analysis of observational studies

Summary: The aim of this study was to assess the clinical efficacy of ceftazidime-avibactam in managing carbapenem-resistant Gram-negative infections in renal patients receiving recommended dosing adjustments. The study found that renal dosing adjustments of ceftazidime-avibactam may be associated with a higher risk of mortality in patients affected by carbapenem-resistant Gram-negative infections.

Summary: This review evaluates the role of pharmacokinetic/pharmacodynamic (PK/PD) optimization of novel beta-lactams in the management of difficult-to-treat resistance (DTR) Gram-negative infections. The findings suggest that optimizing antimicrobial therapy with novel beta-lactams based on PK/PD concepts can maximize clinical efficacy and prevent resistance emergence. Establishing a coordinated multidisciplinary team and implementing personalized antimicrobial exposure optimization can be crucial for the management of DTR Gram-negative infections.

Fast and Simple Liquid Chromatography-Isotope Dilution Tandem Mass Spectrometry Method for Therapeutic Drug Monitoring of Dalbavancin in Long-Term Treatment of Subacute and/or Chronic Infections

Summary: Dalbavancin (DBV) is a long-acting antistaphylococcal lypoglycopeptide increasingly used for long-term treatment of subacute and/or chronic infections. Population pharmacokinetic studies have shown that two 1500mg doses of DBV 1 week apart can effectively treat infections for weeks. This study developed and validated a fast and simple analytical method for measuring DBV concentrations in human plasma microsamples using Liquid Chromatography-Isotope Dilution Tandem Mass Spectrometry (ITD LC-MS/MS) technique.

Summary: In this study, a fast and sensitive LC-ITD-MS/MS method was developed and validated for measuring Cefiderocol (C) in human plasma microsamples as small as 3 microliters. The method was successfully validated based on EMA guidelines and was applied to therapeutic drug monitoring (TDM) of critically ill patients with carbapenem-resistant Gram-negative bacterial infections, optimizing antibiotic therapy.

Summary: This study aimed to develop a population pharmacokinetic/pharmacodynamic (PK/PD) model of valganciclovir for preemptive therapy of cytomegalovirus (CMV) infection in kidney transplant patients. Ganciclovir concentrations and CMV viral loads were retrospectively obtained from kidney transplant patients. Monte Carlo simulations were conducted to assess the probability of attaining viral load targets, and a PK/PD model was devised.

Summary: Acute Bacterial Skin and Skin Structure Infections (ABSSSIs) commonly result in Emergency Department (ED) access and hospital admissions, causing a significant economic burden. Long-acting lipoglycopeptides (LALs) allow for outpatient management of ABSSSI patients who require parenteral therapy but do not need hospitalization. This article discusses the microbiological activity, efficacy, and safety of dalbavancin, critical steps for ABSSSI management in the ED, and the potential advantage of direct/early discharge with dalbavancin.

A novel method to evaluate ceftazidime/avibactam therapy in patients with carbapenemase-producing Enterobactericeae (CPE) bloodstream infections

Summary: This study reports the management of eight patients with bloodstream infections using a continuous infusion of ceftazidime-avibactam. The results show that patients with a lower ceftazidime concentration compared to the minimum inhibitory concentration have a higher risk of microbiological failure and development of resistance.

Summary: This study aimed to investigate the relationship between the maintenance of pharmacokinetic/pharmacodynamic (PK/PD) dalbavancin efficacy thresholds and clinical outcomes. The results showed that maintaining the PK/PD efficacy thresholds of dalbavancin for the majority of the treatment period may be a valuable approach in dealing with long-term staphylococcal infections.
