Summary: Febrile neutropenia (FN) is a common complication in pediatric patients with oncological and hematological diseases, and early detection of bacterial infections is crucial. This study aimed to evaluate presepsin as an early biomarker for bacterial infections during FN. The results showed that presepsin levels were significantly higher in FN patients compared to the control group. However, presepsin was not reliable in differentiating patients with and without bacteremia. Higher presepsin levels were observed in patients with an unfavorable outcome at 48 hours from the onset of fever.

Summary: The diagnostic value of procalcitonin and the prognostic role of PCT clearance were evaluated in neutropenic patients after allogeneic hematopoietic stem cell transplantation. It was found that procalcitonin can be used to detect documented and bloodstream infection, and the inability to decrease procalcitonin by more than 80% within 5-7 days after fever onset predicted poor 100-day survival. A prognostic nomogram combining procalcitonin clearance and clinical parameters effectively predicted the 100-day survival of febrile patients.

The prevalence of SBI varied widely in the included studies, with spectrum bias playing a role. Limitations of the studies included retrospective designs and inconsistent screening for SBI in all subjects.

Summary: This study aimed to investigate the application value of metagenomic next-generation sequencing (mNGS) in children with hematological diseases presenting with Febrile Neutropenia (FN). The results showed that mNGS had a higher detection rate for total and mixed pathogens compared to traditional pathogen detection (TPD). The mNGS positive group had earlier detection, lower mortality, and fewer drug-related adverse events (DRAE) compared to the negative group. Early mNGS can effectively improve the efficacy of pathogen detection and reduce mortality and antibiotic abuse.

The findings suggest that intraoperative cell salvage can reduce perioperative transfusion requirement without affecting inflammatory markers and organ function. The restriction of intraoperative cell salvage in this surgery should be re-evaluated.
