Glissonean approach for hepatic inflow control in minimally invasive anatomic liver resection: A systematic review

Mamoru Morimoto, Federico Tomassini, Giammauro Berardi, Yasuhisa Mori, Chikara Shirata, Mohammed Abu Hilal, Horacio J. Asbun, Daniel Cherqui, Naoto Gotohda, Ho-Seong Han, Yutaro Kato, Fernando Rotellar, Atsushi Sugioka, Masakazu Yamamoto, Go Wakabayashi

Summary: The Glissonean approach has been widely used in MIALR with reported advantages such as shorter operative time, lower blood loss and better peri-operative outcomes. However, clear understanding of Laennec's capsule anatomy is required and the best surgical approach depends on surgeon experience and patient characteristics.

Summary: Minimally invasive anatomic liver resection is still a challenging procedure, but there is a certain degree of consensus among expert liver surgeons. Most experts have over 11 years of experience with MILR and recommend techniques such as creating a demarcation line, indocyanine green negative staining method, and intraoperative ultrasound for identifying IPs.

Summary: This study aimed to investigate the relationship between tumor size and the difficulty of laparoscopic left lateral sectionectomy (L-LLS). Three optimal tumor size cutoffs (40-, 70-, and 100-mm) were identified. As tumor size increased, there was a stepwise increase in the rates of open conversion, operative time, blood loss, intraoperative blood transfusion, Pringle maneuver use, major morbidity, and 30-day readmission.

Summary: This retrospective study evaluated the postoperative and long-term survival outcomes of patients with unresectable or high biological risk HCC and ICC treated with radioembolization (RE) and subsequently rescued by liver surgery with curative intent. The results showed that liver resection after RE is safe and feasible, with good short-term outcomes. Moreover, the long-term outcomes after RE and liver resection were optimal, with a 10-year overall survival rate greater than 50% for HCC and ICC patients. The study also highlighted the efficacy of RE in downstaging or bridging to liver transplantation for HCC patients.

Besselink, Mohammad Abu Hilal

Summary: This retrospective cohort study aimed to compare the outcomes of simultaneous resection (SIMR) for synchronous colorectal liver metastases (sCRLM) in different settings and identify factors associated with unfavorable outcomes. The results showed that SIMR was feasible for patients requiring minor or technically major liver resection. Compared to major resections, minor and technically major resections had lower complication rates and shorter hospital stays. Factors independently associated with an increased risk of severe morbidity included higher American Society of Anesthesiologists grades (>= 3) and undergoing a major liver resection, while undergoing a left-sided colectomy was associated with a decreased risk.

Comparison of phenotypes and outcomes following resection of incidental versus symptomatic pancreatic neuroendocrine tumors

Luis Hurtado-Pardo, Charles E. Breeze, Javier A. Cienfuegos, Alberto Benito, Victor Valenti, Pablo Marti-Cruchaga, Gabriel Zozaya, Fernando Martinez Regueira, Fernando Pardo, Fernando Rotellar

Summary: This study compared the phenotype and oncological outcomes of incidental and symptomatic pancreatic neuroendocrine tumors. The results showed that symptomatic tumors were more frequent in females and younger patients and were diagnosed at a more advanced stage. There were no significant differences in overall and disease-free survival between the two groups. Resection of incidental tumors >1.5-2 cm seems advisable, although each case should be assessed on an individual basis.
