摘要

Liver transplantation (LT) remains the only curative treatment for patients with failed medical treatment for acute-on-chronic liver failure (ACLF). However, the selection criteria for LT in ACLF is ill-defined. Given the scarcity of deceased organs and the inherent risk of living donor hepatectomy, it is mandatory to identify unfavourable prognostic factors for survival in ACLF in order to establish an objective and fair selection criteria for LT, and more importantly to ensure a satisfactory post-transplant outcome. The aim of this article was to review the current evidence on the validity of the existing prognostic models and to evaluate the survival outcomes after LT for ACLF.

Acute-on-chronic liver failure: Where do we stand?

Identifying high-risk cirrhosis patients and managing them carefully are crucial in preventing ACLF.

Summary: Living donor liver transplantation (LDLT) is more common in Asia than in North America, where living donor kidney transplantation, including paired exchanges for ABO-incompatible pairs, is prevalent. The first liver paired exchange (LPE) in the United States, completed over two consecutive days, was initiated by a non-directed donor and benefited an ABO-incompatible pair. This innovative approach demonstrates the feasibility of LPE in centers with experience in LDLT, offering new possibilities for transplantation in cases of ABO incompatibility.

Impact of Tumour Biology on Outcomes of Radical Therapy for Hepatocellular Carcinoma Oligo-Recurrence after Liver Transplantation

Summary: The impact of tumor biology on radical treatment for oligo-recurrence of post-transplant hepatocellular carcinoma (HCC) remains uncertain. This retrospective study found that early recurrence, liver recurrence, and high levels of AFP upon recurrence were predictors of mortality. Radical treatment was associated with improved post-recurrence survival in patients with early and liver recurrence, but not in patients with high AFP levels. The survival benefits of radical therapy were reduced in early recurrence and high AFP, but remained similar among patients with and without liver recurrence. The presence of multiple biological risk factors diminished the survival benefit of radical therapy.

Summary: This study aimed to determine the prevalence of de novo steatosis and MAFLD in liver transplant patients. The results showed that post-transplant de novo graft steatosis is common in Chinese patients, with the majority classified as MAFLD. The development of graft steatosis was not associated with an increase in graft fibrosis but was associated with worse metabolic control and graft dysfunction.

Post-transplant inflammatory cytokine signature adds value for predicting tumor recurrence after liver transplantation for hepatocellular carcinoma

Summary: This study found that post-transplant cytokines CCL11, IFN alpha 2, and IL17A are significant predictors of tumor recurrence and survival after liver transplantation for hepatocellular carcinoma. A prediction model including these cytokines, the UCSF criteria, and pre-transplant AFP accurately predicted post-transplant tumor recurrence and guided refinements in surveillance and therapeutic strategies.

Defining the surgical management for non-colorectal liver metastases

Summary: This study compared the survival outcomes of patients with resection of non-colorectal liver metastases (NCLM) versus colorectal liver metastases (CLM) and identified prognostic factors for resection of NCLM. The results showed that surgical resection of NCLM had comparable survival outcomes with CLM, and the indication of liver resection could be expanded to NCLM with appropriate patient selection.
