Liver transplantation (LT) remains the only curative treatment for patients with failed medical treatment for acute-on-chronic liver failure (ACLF). However, the selection criteria for LT in ACLF is ill-defined. Given the scarcity of deceased organs and the inherent risk of living donor hepatectomy, it is mandatory to identify unfavourable prognostic factors for survival in ACLF in order to establish an objective and fair selection criteria for LT, and more importantly to ensure a satisfactory post-transplant outcome. The aim of this article was to review the current evidence on the validity of the existing prognostic models and to evaluate the survival outcomes after LT for ACLF.

A snapshot of the 2020 conception of anatomic liver resections and their applicability on minimally invasive liver surgery. A preparatory survey for the Expert Consensus Meeting on Precision Anatomy for Minimally Invasive HBP Surgery

Ruben Ciria, Giammauro Berardi, Hitoe Nishino, Albert C. Y. Chan, Rawisak Chanwat, Kuo-Hsin Chen, Yajin Chen, Tan To Cheung, David Fuks, David A. Geller, Yukio Iwashita, Rong Liu, Santiago Lopez-Ben, Masakazu Yamamoto, Go Wakabayashi

Summary: The survey aimed to analyze liver surgeons' practices in liver resections and their understanding of anatomic procedures in the context of minimally invasive liver surgery (MILS). Results revealed that most surgeons performing MILS have less than 10 years of experience and low utilization of new techniques. While the majority agree with the concept of anatomic resections, the application of complex anatomic techniques in MILS is not widespread in practice.

Adjuvant transarterial chemotherapy for margin-positive resection of hepatocellular carcinoma-a propensity score matched analysis

Alvina Jada Fok, Wong Hoi She, Ka Wing Ma, Simon H. Y. Tsang, Wing Chiu Dai, Albert C. Y. Chan, Chung Mau Lo, Tan To Cheung

Summary: Adjuvant transarterial chemotherapy may offer survival benefits to hepatocellular carcinoma patients, particularly those with positive surgical margins. The patient and disease characteristics of the group receiving adjuvant therapy were largely comparable to those who did not receive chemotherapy.

Verifying the Benefits of Radical Treatment in Posttransplant Hepatocellular Carcinoma Oligo-recurrence: A Propensity Score Analysis

Kin Pan Au, James Yan Yue Fung, Wing Chiu Dai, Albert Chi Yan Chan, Chung Mau Lo, Kenneth Siu Ho Chok

Summary: This study showed that radical treatment for hepatocellular carcinoma (HCC) oligo-recurrence after liver transplantation can lead to improved survival outcomes. Patients receiving palliative treatment had earlier recurrences and more extensive disease compared to those who received radical treatment.

Summary: Minimally invasive anatomic liver resection is still a challenging procedure, but there is a certain degree of consensus among expert liver surgeons. Most experts have over 11 years of experience with MILR and recommend techniques such as creating a demarcation line, indocyanine green negative staining method, and intraoperative ultrasound for identifying IPs.

Impact of Tumour Biology on Outcomes of Radical Therapy for Hepatocellular Carcinoma Oligo-Recurrence after Liver Transplantation

Kin-Pan Au, James Yan-Yue Fung, Wing-Chiu Dai, Albert Chi-Yan Chan, Chung-Mau Lo, Kenneth Siu-Ho Chok

Summary: The impact of tumor biology on radical treatment for oligo-recurrence of post-transplant hepatocellular carcinoma (HCC) remains uncertain. This retrospective study found that early recurrence, liver recurrence, and high levels of AFP upon recurrence were predictors of mortality. Radical treatment was associated with improved post-recurrence survival in patients with early and liver recurrence, but not in patients with high AFP levels. The survival benefits of radical therapy were reduced in early recurrence and high AFP, but remained similar among patients with and without liver recurrence. The presence of multiple biological risk factors diminished the survival benefit of radical therapy.

Association of Multi-Phasic MR-Based Radiomic and Dosimetric Features with Treatment Response in Unresectable Hepatocellular Carcinoma Patients following Novel Sequential TACE-SBRT-Immunotherapy

Summary: This study investigates the association between pre-treatment multi-phasic MR-based radiomics and treatment response to a novel therapeutic regimen for unresectable HCC. The study identifies specific radiomic features that are associated with treatment response and demonstrates the potential for pre-treatment identification of responsive HCC patients. The findings highlight the importance of personalized oncology for HCC management.

Summary: This study aimed to determine the prevalence of de novo steatosis and MAFLD in liver transplant patients. The results showed that post-transplant de novo graft steatosis is common in Chinese patients, with the majority classified as MAFLD. The development of graft steatosis was not associated with an increase in graft fibrosis but was associated with worse metabolic control and graft dysfunction.

Post-transplant inflammatory cytokine signature adds value for predicting tumor recurrence after liver transplantation for hepatocellular carcinoma

Kevin Tak-Pan Ng, Jiang Liu, Oscar Wai-Ho Yeung, Li Pang, Hoi Chung Shiu, Hui Liu, Xin Xiang Yang, Albert Chi-Yan Chan, Tiffany Cho-Lam Wong, Chung Mau Lo, Kwan Man

Summary: This study found that post-transplant cytokines CCL11, IFN alpha 2, and IL17A are significant predictors of tumor recurrence and survival after liver transplantation for hepatocellular carcinoma. A prediction model including these cytokines, the UCSF criteria, and pre-transplant AFP accurately predicted post-transplant tumor recurrence and guided refinements in surveillance and therapeutic strategies.

Defining the surgical management for non-colorectal liver metastases

Yan Luk, Wong Hoi She, Simon H. Y. Tsang, Wing Chiu Dai, Albert C. Y. Chan, Tan To Cheung, Chung Mau Lo

Summary: This study compared the survival outcomes of patients with resection of non-colorectal liver metastases (NCLM) versus colorectal liver metastases (CLM) and identified prognostic factors for resection of NCLM. The results showed that surgical resection of NCLM had comparable survival outcomes with CLM, and the indication of liver resection could be expanded to NCLM with appropriate patient selection.
