Summary: The study indicated that intraoperative endoluminal pyloromyotomy during pylorus-preserving pancreaticoduodenectomy was associated with a reduced risk for delayed gastric emptying. Additionally, the presence of an intra-abdominal complication was identified as an independent risk factor for delayed gastric emptying.

Summary: This study found a higher incidence of delayed gastric emptying (DGE) in patients undergoing pylorus-preserving pancreaticoduodenectomy (PpPD) compared to pylorus-resecting pancreaticoduodenectomy (PrPD) by a single surgeon. Pylorus preservation and clinically relevant postoperative pancreatic fistula were identified as risk factors for DGE.

Summary: This study aims to investigate the effect of intraoperative endoluminal pyloromyotomy on delayed gastric emptying (DGE) after pylorus-preserving pancreaticoduodenectomy (ppPD). The randomized trial will compare the rate of DGE and other secondary endpoints between patients who receive endoluminal pyloromyotomy and patients who undergo atraumatic stretching of the pylorus.

Summary: This meta-analysis compared the outcomes of intraoperative pyloric drainage versus no intervention in patients undergoing esophagectomy. The results showed that intraoperative pyloric drainage can reduce delayed gastric emptying but has similar post-operative outcomes in terms of complications and mortality.

Summary: The resistance of the pyloric channel is influenced by pressure gradient, flow behavior index, and channel diameter. Non-zero pressure difference is essential to drive flow through the channel, and the extent of pressure difference is linearly related to transpyloric flow rate. The resistive function of the pyloric channel is observed at a higher occlusion where there is a development of higher pressure barrier.

Summary: A clinical trial compared the incidence and severity of DGE between classical Whipple and PPPD, finding no significant differences. In addition to DGE, there were no significant differences between the two surgical techniques in terms of postoperative complications, length of hospital stay, and nutritional status.

Summary: This study compared the clinical outcomes and incidence of delayed gastric emptying (DGE) between open and robotic-assisted pancreatoduodenectomy (PpPD) and found that the robotic approach was associated with a lower incidence of DGE and a shorter hospital stay.

Summary: This study evaluated the micronutrient status after pancreaticoduodenectomy with a 12-month follow-up. Results showed deficiencies in vitamin D, B1, B6, and iron in over 50% of patients over time. Significant differences were found between the groups in cholesterol, HDL, and selenium levels.

Summary: This study investigated the association between intraoperative end-tidal carbon dioxide (EtCO2) levels and length of hospital stay (LOS) in patients undergoing pylorus-preserving pancreaticoduodenectomy (PPPD). The results showed that hypocapnia was an independent risk factor for longer LOS, with low EtCO2 levels during general anesthesia being associated with increased LOS in PPPD patients.

Summary: Pyloric EsoFLIP controlled dilation shows value in the treatment of gastroparesis, with significant improvements in postprocedural gastric emptying time and symptoms. Long-term follow-up and comparative trials are warranted to assess efficacy.

Summary: Implementation of risk-stratified pancreatectomy clinical pathways (RSPCPs) can significantly reduce the risk of delayed gastric emptying (DGE) after pancreaticoduodenectomy (PD). Factors such as postoperative abscess and non-white patients are associated with higher rates of DGE. Early nasogastric tube removal within 48 hours is associated with lower rates of DGE.

Summary: Recent research indicates that pyloric distensibility may serve as an important indicator for evaluating patients with gastroparesis, as its decrease is strongly correlated with gastric emptying and symptoms. Furthermore, pyloric distensibility may also predict the efficacy of endoscopic techniques targeting the pylorus.

Summary: The study shows that smokers have a lower incidence of delayed gastric emptying syndrome (DGE) after pancreatoduodenectomy and tolerate solid food intake more quickly. Smokers are typically younger, predominantly male, with a higher prevalence of pulmonary conditions and alcohol abuse.

Summary: Lichen planus is a chronic disease that affects the skin, appendages, and mucous membranes. The etiology of the disease is not fully understood, but immune-mediated mechanisms are recognized, with genetic susceptibility and environmental factors playing a role. The immunopathogenesis involves cell-mediated cytotoxicity, particularly cytotoxic T lymphocytes, along with the involvement of other immune cells and inflammatory pathways.

Summary: This case series presented four mechanically ventilated patients with severe bilateral COVID-19 pneumonia complicated by ARDS, who were treated using the segmental lung recruitment maneuver. The results showed that in three out of four patients, there was an improvement in the PaO2/FiO(2) ratio and the lung ultrasound scoring system after 48 hours of segmental lung recruitment. In conclusion, the segmental lung recruitment maneuver is an effective method for lung recruitment in patients with bilateral COVID-19 pneumonia complicated by ARDS.
