Patients who ingest toxic substances may require extracorporeal removal of the poisons or their toxic metabolites if native renal clearance is not sufficient because of acute kidney injury, acuity of symptoms, or burden of toxin. Here, a case is presented, and the literature on renal replacement therapy in the event of acute intoxication is reviewed. Extracorporeal therapy efficacy is examined in terms of the characteristics of the toxin (molecular size, charge, protein, or lipid binding); the patient (body habitus and volume of distribution); and the process (membrane effects on extraction ratios and sieving, role of blood, and dialysate flow rates). The choice of extracorporeal therapy and hemodialysis prescriptions for specific poisonings are discussed.

Summary: The lysine methyltransferase SMYD2 is highly expressed in renal fibrosis and plays a critical role in converting epithelial cells to a profibrotic phenotype, activating renal interstitial fibroblasts, and promoting renal fibrogenesis. Inhibition of SMYD2 with AZ505 could potentially be a target for treating chronic fibrosis in kidney disease.

Nephrotoxicity of immune checkpoint inhibitor therapy: a pharmacovigilance study

Summary: By utilizing a pharmacovigilance approach, increased odds of nephritis were found with ICI therapy. Pembrolizumab, nivolumab, and the combination of ipilimumab + nivolumab showed the highest odds of nephritis among the examined ICI drugs.

Summary: In this study, the role and mechanism of PRC2 in acute kidney injury (AKI) were investigated. The results showed that inhibition of PRC2 activity by targeting EED could protect against cisplatin-induced AKI by promoting the survival and proliferation of renal tubular cells and inhibiting inflammatory response.
