Cellular (FLICE) like inhibitory protein (cFLIP) expression in diffuse large B-cell lymphoma identifies a poor prognostic subset, but fails to predict the molecular subtype

Immunohistochemistry can sub-classify diffuse large B-cell lymphoma (DLBCL) into germinal centre B-cell like (GCB) and non-GCB subtypes. The latter consists predominately of the activated B-cell like subgroup in which nuclear factor kappa-B activation is its characteristic. Expression of cellular caspase 8 (FLICE)-like inhibitory protein (cFLIP), a caspase 8 homologue, is regulated by nuclear factor kappa-B signalling, and it is the main inhibitor of Fas ligand activated apoptosis. To determine if cFLIP expression was confined to non-GCB subtype, we studied 66 cases of DLBCL. cFLIP expression showed no significant correlation to DLBCL subtypes (GCB or non-GCB) but was associated with a worse clinical outcome. For cFLIP positive and negative patients, the five-year event free survival was 20 and 31%, respectively (p?=?0.049), and the five-year overall survival was 20 and 57%, respectively (p?=?0.041). Summary: A subset of nodular lymphocyte-predominant Hodgkin lymphoma patients have variant histological patterns, which are associated with advanced disease stage and increased recurrence risk. Analysis of histopathology reports from 33 adult patients revealed that 39% had variant histology. Variant histology was significantly associated with stage IV disease.

Summary: Despite the effectiveness of immuno-chemotherapy, there is a significant rate of relapse or refractory disease in patients with DLBCL. Our study focuses on the changes in gene expression profiles (GEP) that accompany DLBCL relapse, and we have developed a 30-gene discriminator that can identify clinically distinct high and low-risk subgroups in DLBCL patients. This signature has also been found to predict better outcomes in patients treated with ibrutinib-R-CHOP.

Summary: In this retrospective study, 160 patients with high-grade B-cell lymphoma, not otherwise specified (HGBL-NOS), were examined to determine their characteristics and outcomes. The study found that HG BL-NOS tumors are heterogeneous and most patients presented with advanced stage disease and high-risk clinical factors. Poor performance status, high lactate dehydrogenase, and a dual-expressor immunophenotype were identified as main prognostic factors for progression-free survival and overall survival.

Abnormal bone marrow findings in patients following treatment with chimeric antigen receptor-T cell therapy

Summary: This study retrospectively observed bone marrow abnormalities in 259 patients receiving CAR-T cell immunotherapy. The study found that 25.5% of patients showed severe marrow hypocellularity and 6.2% showed serous atrophy at a median of 35.5 days post-infusion. Additionally, the study found correlations between bone marrow pathology findings and disease burden and survival rates.

Summary: This study validated the use of FusionPlex Pan-Heme panel analysis for routine clinical testing, which can sensitively and rapidly identify a wide range of gene fusions for precise diagnosis and prognostication. The validation results showed good concordance between the FusionPlex panel and other methods. In an independent cohort, this analysis reliably detected gene fusions, contributing to accurate classification of hematologic neoplasms and discovery of new fusion partners.

Summary: We report a rare and aggressive case of gamma delta T-cell lymphoproliferative disorder/lymphoma that presented in the skin without the expected cytotoxic markers but with increased expression of CD5, CD20, CD79a, CD30, and PD-I. Monoclonal TCR-gamma gene rearrangement was identified. The clinical, morphologic, and immunophenotypic features of this case do not fit into the current classifications.
