Summary: This study aimed to quantify fatty infiltration and magnetization transfer ratio in individual muscles of patients with symptomatic and asymptomatic TTR-FAP using magnetic resonance imaging. The results showed elevated fatty infiltration and decreased magnetization transfer ratio in the lower limbs of symptomatic TTR-FAP patients. In asymptomatic patients, elevated fatty infiltration in the gastrocnemius lateralis muscle was observed. Fatty infiltration was significantly correlated with disease duration and neuropathy impairment scores.

Summary: The study evaluated the long-term prognosis of a large cohort of asymptomatic patients with Brugada ECG pattern. The entire population of asymptomatic patients with BrECG exhibited a relatively low event rate per year, with those having spontaneous type-1 BrECG and positive EPS showing higher risk. Asymptomatic patients with drug-induced-only BrECG had minimal arrhythmic risk, but ongoing follow-up was recommended to detect the appearance of spontaneous type-1 BrECG pattern.

Summary: In this study, features of FAP with E61K TTR substitution were characterized, showing low amyloidogenic properties in vitro and in vivo. Apoptotic cells were observed in the nerve of patients, and chromatin condensation was confirmed in Schwann cells by electron microscopy. These findings suggest that E61K TTR may not contribute significantly to amyloid fibril formation, but Schwann cell apoptosis could be related to neurodegeneration in FAP.

The study found that the screening program identified a certain proportion of major cardiac conditions in the athletes. Summary: This study reveals that the main causes of overdiagnosis of Long QT syndrome (LQTS) are prolonged QTc interval due to vasovagal syncope, mistaken positive genetic test results, unrelated family history of sudden cardiac death (SCD), isolated/transient QT prolongation, and misinterpretation of QTc interval. Understanding these determinants can increase awareness and provide critical guidance to reduce the burden of overdiagnosed LQTS.

The model effectively identified the risk for SCD in Asians without a history of coronary artery disease or low left ventricular ejection fraction.

Summary: This study aims to investigate the prognostic value of early repolarization pattern (ERP). The results showed that time-varying ERP is associated with increased risks of sudden cardiac death (SCD) and cardiovascular death. Compared with individuals with consistently normal ECG findings, those with new-onset ERP or consistent ERP experienced increased risks of developing SCD and cardiovascular death. Time-varying ERP in women, White subjects, and those with higher J-wave amplitudes appeared to indicate poorer cardiovascular outcomes.

Summary: This study assessed the outcomes of patients with post-resuscitation shock after out-of-hospital cardiac arrest, based on the choice of continuous intravenous vasopressor (epinephrine or norepinephrine). The results showed that the use of epinephrine was associated with higher all-cause and cardiovascular-specific mortality compared to norepinephrine infusion. Therefore, until more data becomes available, intensivists may prefer norepinephrine over epinephrine for the treatment of post-resuscitation shock.

Summary: The aim of this study is to investigate the impact of direct oral anticoagulant (DOAC) management on the incidence of pocket haematoma in patients undergoing pacemaker or implantable cardioverter-defibrillator implantation. The results of a large multicenter prospective observational study showed that the incidence of clinically relevant haematoma was low in patients with DOAC interruption, indicating that DOAC management can effectively reduce the risk of pocket haematoma.
