Effect of partial inhibition of fatty acid oxidation by trimetazidine on whole body energy metabolism in patients with chronic heart failure

Objective Trimetazidine may have beneficial effects on left ventricular (LV) function in patients with systolic heart failure. The authors assessed whether long-term addition of trimetazidine to conventional treatment could improve, along with LV function, resting whole body energy metabolism in patients with chronic systolic heart failure. Design Single blind randomised study. Setting University Hospital. Patients 44 patients with systolic heart failure receiving full medical treatment. Interventions Indirect calorimetry and two-dimensional echocardiography at baseline and after 3 months. Main outcome measures Whole body resting energy expenditure (REE), percentage of predicted REE, LV ejection fraction (EF), NYHA class, quality of life. Results Trimetazidine increased EF compared with conventional therapy alone (from 35 +/- 8% to 42 +/- 11% vs from 35 +/- 7% to 36 +/- 6%; p=0.02, analysis of variance for repeated measures). NYHA class and quality of life also improved compared with conventional therapy (p<0.0001). REE (from 1677 +/- 264 to 1580 +/- 263 kcal/day) and percentage of predicted REE (based on the Harris-Benedict equation: from 114 +/- 10% to 108 +/- 9%) decreased in the trimetazidine group, but not in the control group (REE from 1679 +/- 304 to 1690 +/- 337 kcal/day and percentage of predicted REE from 113 +/- 12% to 115 +/- 14%). The variation was different between groups (p=0.03 and 0.023, respectively). Conclusions In patients with systolic heart failure, improvement in functional class and LV function induced by middle-term trimetazidine therapy is paralleled by a reduction in whole body REE. The beneficial cardiac effects of trimetazidine may be also mediated by a peripheral metabolic effect.

Prolonged use of proton pump inhibitors and risk of type 2 diabetes: Results from a large population-based nested case-control study

Peripheral artery disease and all-cause and cardiovascular mortality in patients with NAFLD

Summary: This study aimed to assess the prevalence of peripheral arterial disease (PAD) in patients with non-alcoholic fatty liver disease (NAFLD) and its association with all-cause and cardiovascular disease (CVD) mortality. The results showed that PAD was associated with a higher risk of all-cause and cardiovascular mortality, supporting the current guidelines recommending CVD screening in NAFLD patients.

Trends in prevalence of probable fibrotic non-alcoholic steatohepatitis in the United States, 1999-2016

Summary: Using data from the National Health and Nutrition Examination Survey, the authors applied the Fibrotic NASH Index (FNI) to estimate the prevalence of fibrotic non-alcoholic steatohepatitis (NASH) in the US population. They found that the prevalence of probable fibrotic NASH was 8.4%, with higher rates in males and Hispanic participants. The study also revealed an increasing trend in NASH prevalence, which is associated with rising rates of obesity and type 2 diabetes.

Noninvasive identification of probable fibrotic nonalcoholic steatohepatitis across the spectrum of glucose tolerance in the United States

Summary: This study aims to report the prevalence of fibrotic NASH in the general US population and evaluate the performance of the Fibrotic NASH Index (FNI). The study found that FNI had good accuracy in predicting probable fibrotic NASH in the overall population and in individuals with diabetes. This suggests that FNI is a simple and reliable test for screening NASH.

Liver stiffness is associated with all-cause mortality in patients with NAFLD: A systematic review and meta-analysis

Summary: This systematic review and meta-analysis found an association between liver stiffness measurement (LSM) obtained through vibration-controlled transient elastography (VCTE) and all-cause mortality in patients with nonalcoholic fatty liver disease (NAFLD). The analysis showed that higher LSM, indicating liver fibrosis, is independently associated with a higher mortality risk in NAFLD patients.

Differential Association of Sex Hormones with Metabolic Parameters and Body Composition in Men and Women from the United States

Summary: Sex hormones have different impacts on body composition in men and women. This study aimed to evaluate the association between testosterone, estradiol, and body fat distribution in the general population. The results showed that testosterone was positively associated with lean body mass and negatively associated with fat mass and the android/gynoid ratio in men, while estradiol showed the opposite trend. Among women, testosterone did not have an impact on body composition, but estradiol was positively associated with lean mass and negatively associated with fat mass. These findings highlight the varying effects of sex hormones on specific fat depots between men and women.

Nonalcoholic Fatty Liver Disease in Patients with Type 2 Diabetes: Screening, Diagnosis, and Treatment

Summary: Nonalcoholic fatty liver disease (NAFLD), also known as metabolic dysfunction-associated steatotic liver disease (MASLD), is common in patients with type 2 diabetes (T2D) and can lead to advanced liver fibrosis. Patients with T2D have an increased risk of liver-related complications and mortality. Screening for NAFLD/MASLD and advanced liver fibrosis is recommended in T2D patients. While there are no approved drugs for NAFLD/MASLD, certain antidiabetic drugs have shown positive effects on liver health. Diabetologists have the opportunity to actively treat NAFLD/MASLD and potentially change its natural course.

Impact of the new definition of metabolic dysfunction-associated fatty liver disease on detection of significant liver fibrosis in US adolescents

Summary: This study aimed to evaluate the prevalence of MAFLD among US adolescents and investigate whether the new MAFLD definition can identify individuals with more advanced liver disease. The study found that most US adolescents with elastographic evidence of steatosis meet the criteria for MAFLD, but these criteria do not appear to improve the detection of subjects with more advanced liver disease.

Soluble alpha-Klotho levels, glycemic control and renal function in US adults with type 2 diabetes
