Improvements in health care are slow, in part because doctors and nurses lack skills in quality improvement, patient safety, and interprofessional teamwork. This article reports on the Retooling for Quality and Safety initiative of the Josiah Macy Jr. Foundation and the Institute for Healthcare Improvement, which sought to integrate improvement and patient safety into medical and nursing school curricula. In one academic year, 2009-10, the initiative supported new learning activities (87 percent of which were interprofessional, involving both medical and nursing students) in classrooms, simulation centers, and clinical care settings that involved 1,374 student encounters at six universities. The work generated insights-described in this article-into which learning goals require interprofessional education; how to create clinically based improvement learning for all students; and how to demonstrate the effects on students' behavior, organizational practice, and benefits to patients. A commonly encountered limiting factor for the programs was the lack of a critical mass of clinically based faculty members who were ready to teach about the improvement of care. What's more, the paucity of robust evaluation strategies for such programs suggests a future research agenda that deserves to be funded.

Summary: Most nursing journals require educational studies and QI projects to undergo IRB or other ethics committee review, especially those involving the professional development of nurses. Any determination of exemption should be made by the IRB or research ethics committee, not by the investigator.

Summary: This study aimed to create a fully integrated nurse-led model of a psychiatric nurse practitioner and behavioral health care team within primary care. Through the Rapid Cycle Quality Improvement model, three task forces were established and four interventions were implemented. The results showed that this quality improvement effort can effectively support primary care providers and improve access to psychiatric services for more patients.

Summary: Lack of access to birth facilities and maternity care providers has contributed to rising US maternal mortality and morbidity rates, especially among women in rural areas. Evidence supports the increased use of midwives as a potential solution for access-to-care issues. This study examines the practice environment for Certified Nurse-Midwives (R) in Colorado and suggests policy recommendations for expanding midwifery practices.

Including Voices of Adolescents With Chronic Conditions in the Redesign of Children's Mental Health Systems: Implications for Resource Allocation

Summary: This study aimed to explore the perspectives of adolescents with chronic conditions on mental health system redesign to improve outcomes. Through semi-structured interviews with 17 adolescents, the study found that they need to be heard, they need someone they can really talk to and trust, they need the mental health system to reach out and check in on them, and the school nurse is primarily for physical illness. The discussion highlights the need for mental health system redesign for adolescents with chronic conditions and suggests future research to test innovative healthcare delivery models to reduce mental health disparities in this vulnerable population.
