The Cleveland Clinic Health System (CC) is searching for a doctoral level faculty member to join its multidisciplinary Department of Quantitative Health Sciences on the Cleveland, Ohio main campus. This is a full-time position consisting of 70% designing/analyzing clinical trials and observational studies with investigators from the CC’s Digestive Disease and Surgery Institute (DDSI), and 30% independent research. A doctoral degree with expertise in biostatistics and/or epidemiology is required. Here is a look at 76 patients undergoing D2 gastrectomy at a single large Indian hospital. All patients received adjuvant chemo (n=33) with 5FU / capecitabine plus oxaliplatin or cisplatin, with docetaxel from FLOT conspicuously missing, or adjuvant conventionally-fractionated radiation with concurrent 5FU. In this experience, those receiving chemoradiation achieved a significantly longer recurrence-free survival (83%) than those receiving chemo alone (63%). At the same time, overall survival was similar despite those receiving chemoration having much worse tumor characteristics. This led the authors to humbly question if there should be a radiation comeback for gastric cancer in the era of modern techniques such as IMRT that was exclusively used here. The CD30 antibody-drug conjugate brentuximab vedotin is newly approved for use in classical Hodgkin lymphoma as well as mycosis fungoides, leaving the predictable gap in data on whether it is safe to administer radiation in close proximity. A retrospective review of outcomes with radiation delivered within three weeks of brentuximab vedotin (n=44) suggests the addition of radiation, particularly at higher doses, may contribute to grade 2+ heme toxicity, as experienced by roughly 1 in 5 patients. As might be expected, the recurrence patterns in EORTC 22922 were influenced by the extent of surgery and radiation with a higher rate of local recurrence after breast conservation surgery and a higher risk of regional recurrence without regional nodal irradiation. Interstitial lung disease is a harbinger for scary outcomes with thoracic radiation. This retrospective look at outcomes for 113 patients receiving now standard definitive chemoradiation followed by maintenance durvalumab for locally-advanced non-small cell lung cancer (NSCLC) at a large Japanese center reports the presence of interstitial lung abnormalities (ground-glass attenuation, fine reticular opacity, microcysts, or honeycombing) at pretreatment baseline were a primary predictor of worse overall and cancer specific survival. This is likely due to the fact that over half of these patients (9/17) discontinued durvalumab early due to pneumonitis as compared to 18% of the remaining patients. Exploring modifiable risk factors, the authors suggest striving for stricter dose constraints for these patients and propose a mean lung dose < 12.2 Gy and lung V20 < 22%. Infertility is one of the most consequential long term toxicities of several oncologic therapies used in premenopausal women and girls. Here is an excellent summary on the contemporary options available across a myriad of scenarios from taking a break during maintenance endocrine therapy to preemptive cryopreservation of oocytes/embryos or ovarian tissue (when prepubertal) prior to initiation of follicle-damaging chemotherapy or radiation. Future directions are also touched on, including in vitro follicle maturation, ovarian grafting, ovarian-protecting agents, and newly-discovered ovarian stem cells. While there may remain some debate over who qualifies for PSMA-(re)staging scans, most insurances will agree to it upon biochemical recurrence following prostatectomy, assuming the PSA is at least 0.2. Will this new imaging information alter the mountains of data we have on predicting outcomes with salvage radiation? This large international retrospective cohort of over one thousand men receiving salvage radiation after re-staging PSMA scan demonstrates all the usual suspects still predict subsequent biochemical failure: pre-salvage PSA (HR 1.80), grade group 5 (HR 2.39), pT3b+ (HR 1.91), R0 resection (HR 0.60), addition of ADT (HR 0.49), salvage radiation dose >70 Gy EQD2 (HR 0.44). Here is a Swiss prospective evaluation of 424 consecutive women receiving whole breast irradiation of 40 Gy in 15 fractions for stage 0-III breast cancer. The kicker is they all got a simultaneous daily boost to the lumpectomy cavity to a total of 48 Gy. Of note, comprehensive nodal regions were included when indicated using the same fractionation. What’s more, almost all women were treated with 3D techniques with a daily cone down using static fields. The only late grade 3 toxicity was one event of breast edema. Excellent to good cosmesis was achieved in 97% of women out to 4 years. At 3 years, disease-free survival was 96% and locoregional recurrence rate only 0.6%.
