Eligibility
The organization must be classified as a charitable organization under Section 501(c)(3) of the Internal Revenue Code; or classified as an organization under Section 170(c)(1) of the Internal Revenue Code, or have an eligible fiscal agent.The organization must be located in, or providing services in, northeastern Minnesota, northwestern Wisconsin or in the far western portion of Michigan's Upper Peninsula. If you are located outside our service area, you must be able to demonstrate collaborative and/or non-duplicative activities.
Description
The following grant funds are available to community applicants:1. Miller-Dwan Foundation Innovations FundPrograms and services must fit within the grant-making interests and historical specialty areas of the Foundation. Projects may fall along a spectrum from prevention activities, to assistance within the medical community, to community-based programs.2. Van Gorden Fund
- New Initiatives OutreachThe Van Gorden New Initiatives Outreach Fund was established to support and promote new and innovative projects of rehabilitation services, research and education and to foster creativity and excellence in rehabilitation services programming. Programs and projects must show proof of established rehabilitation goals and further the work of Polinsky and Essentia Health / Miller-Dwan Rehabilitation Services.3. Van Gorden Fund - New Initiatives PolinskyThe Fund was established to support and promote new and innovative projects of rehabilitation services, research, and education at Polinsky and to foster creativity and excellence in rehabilitation services programming.
